目的探讨老年人卧位血压与肱踝动脉脉搏波传导速度(baPWV)的关系。方法在参加开滦研究第3次健康体检的人群中,采取整群抽样的方法抽取年龄≥60岁的开滦集团离退休员工3064人,其中符合入选标准纳入统计分析者1336例。采用偏相关分析坐位、卧位、立位即刻、立位3min收缩压、舒张压与baPWV的相关性,多因素Logistic回归分析卧位高血压对baPWV的影响。结果观察对象1336(男性891、女性445)例,年龄(67.6±6.1)岁,baPWV为(1778.78±348.61)cm/s。坐位、卧位、立位即刻、立位3min收缩压、舒张压均与baPWV呈正相关(均P<0.01)。多因素Logistic回归分析显示:校正了立位低血压、立位高血压等因素后,卧位高血压、坐位高血压是baPWV的危险因素(卧位OR=3.10,95%CI2.13~4.51;坐位OR=2.79,95%CI 1.80~4.31)。结论与坐位、立位血压相比,卧位血压与baPWV的联系更密切。
Objective To investigate the relationship between the supine position blood pressure and brachial-ankle pulse wave velocity (baPWV). Methods A total of 3 064 retired staff of Kailuan Group aged 30 or older who took part in the third physical examination of Kailuan were enrolled in the study. Among them, 1,364 were eligible for inclusion in the statistical analysis. Partial correlation analysis of sitting, lying position, stand immediately, stand 3min systolic blood pressure, diastolic blood pressure and baPWV correlation, multivariate Logistic regression analysis of the impact of supine hypertension on baPWV. Results A total of 1336 cases (891 males and 445 females) were observed. The mean age of patients (67.6 ± 6.1) and baPWV was (1778.78 ± 348.61) cm / s. Sit, supine position, stand immediately, stand 3min systolic blood pressure, diastolic blood pressure and baPWV were positively correlated (all P <0.01). Multivariate Logistic regression analysis showed that lying position hypertension and sitting hypertension were the risk factors of baPWV after correction of orthostatic hypotension and orthostatic hypotension (supine OR = 3.10,95% CI 2.13-4.51; Sitting OR = 2.79, 95% CI 1.80 ~ 4.31). Conclusion Compared with sitting position and standing blood pressure, the relationship between lying position blood pressure and baPWV is more close.
