目的 探讨超声引导下醋酸注射治疗肝肿瘤的效果。方法 在超声引导下对 2 0只家兔肝组织注射不同浓度及剂量的醋酸 ,分别于即刻、1d、3d及 1周后观察超声图像 ,之后处死动物进行病理检查 ,同时测量其凝固区直径。采用兔肝VX2 肿瘤模型 8只 ( 10个结节 )进行醋酸瘤内注射。注射后即刻及注射后 1d处死动物观察其病理变化。另有对照组动物 6只 ( 8个结节 )同期处死 ,比较肿瘤生长情况。结果 注射醋酸后动物肝脏立即发生凝固性坏死 ,坏死区呈椭球形。坏死区大小与其浓度及剂量呈正比。超声引导下醋酸瘤内注射 2 4h后 8个结节肿瘤细胞发生完全凝固性坏死 ,而注射后 12h内 2个结节肿瘤原发灶大部分发生变性及凝固性坏死 ,边缘有少量肿瘤残存 ,而对照组肿瘤结节的平均直径为 1.3cm ,两组差异有显著性意义。结论 醋酸对兔肝VX2 肿瘤有极强的杀伤作用 ,治疗后 2 4h完全坏死。
Objective To investigate the effect of ultrasound-guided acetic acid injection in the treatment of liver tumors. Methods Ultrasound-guided injection of acetic acid in different concentrations and doses of liver tissue was performed on 20 rabbits. Ultrasound images were observed immediately, 1d, 3d and 1 week later. Animals were sacrificed for pathological examination, and the coagulation zone diameter was measured. Eight intratumoral injections (10 nodules) of rabbit liver VX2 tumors were performed. Animals were sacrificed immediately after injection and 1d after injection to observe their pathological changes. Six animals (eight nodules) in the control group were sacrificed at the same time to compare tumor growth. Results The coagulation necrosis occurred immediately after the injection of acetic acid. The necrotic area was ellipsoidal. The size of the necrotic area is proportional to its concentration and dose. Ultrasound-guided acetic acid intratumoral injection of tumor cells in 8 nodules after 24 hours of complete coagulation necrosis, and within 12h after injection of the two primary tumors of the primary tumor degeneration and coagulation necrosis, the edge of a small amount of tumor remnants, The average diameter of tumor nodules in the control group was 1.3 cm, and the difference between the two groups was significant. Conclusion Acetic acid has a strong killing effect on rabbit liver VX2 tumors, and complete necrosis at 24 hours after treatment.
