目的通过中性粒细胞碱性磷酸酶(NAP)染色,探讨各种血液病人NAP活性和阳性指数用来诊断及鉴别诊断的价值,以备早期及时诊断和治疗。方法应用快速偶氮偶联法对1103例,50多种不同疾病血液病人进行NAP活性和阳性指数检测,并分组配对与正常组进行参比检测,经统计学处理,算出相关性、敏感性和特异性。结果在贫血组中,其中再障(AA),AA-PNH综合征,酶的活性明显高于其他贫血和正常组,相互之间有显著差异,P<0.01;在骨髓增殖性疾病(MPD)组中,真性红细胞增多征(PV),骨髓纤维化(MF)酶的活性较其他增殖性疾病和正常组明显增高,P<0.01;白血病组,急淋(ALL),慢淋(CLL),毛白(HCL),浆白(PCL),酶的活性较其他白血病和正常组明显增高,P<0.01,而急非淋M1、M2、M4、全髓性白血病(PMS)酶的活性最低;肿瘤组中,骨髓瘤(MM)酶的活性增高最明显,其次是非霍奇金淋巴瘤(NHL),而恶性组织细胞病(MH)酶的活性极度减低或消失为零,相互之间有显著差异P<0.01;在自身免疫性血小板减少性紫癜(AITP),全血细胞减少、粒细胞减少、血小板减少和脾亢等疾病中,除了AITP较正常不同程度增高,但经统计学处理,无明显差异,P>0.05;结论NAP对血液病不同病期,治疗前后酶的活性有明显差异,可作血液病人早期诊断及鉴别诊断敏感性和特异性诊断指标。
Objective To investigate the value of NAP activity and positive index of various blood patients for the diagnosis and differential diagnosis by neutrophil alkaline phosphatase (NAP) staining in order to diagnose and treat early. Methods Rapid azo coupling method was used to detect the activity and positive index of NAP in 1103 cases and more than 50 kinds of blood diseases of different diseases. The patients were matched with the normal group for reference test. The correlation, sensitivity and Specificity. Results In the anemia group, the activity of AA and AA-PNH syndromes and enzymes were significantly higher than those of other anemia and normal controls (P <0.01). In the case of myeloproliferative disorders (MPD) In the group, the activity of polycythemia vera (PV) and myelofibrosis (MF) was significantly higher than that of other proliferative diseases and normal controls (P <0.01). The leukemia group, ALL, CLL, The activities of HCL and PCL were significantly higher than those of other leukemias and normal controls (P <0.01), while the activity of PML was the lowest (P <0.05) In the tumor group, the activity of myeloma (MM) enzyme increased most obviously, followed by non-Hodgkin’s lymphoma (NHL), while the activity of malignant histiocytosis (MH) enzyme was reduced or disappeared to zero with significant difference The difference was significant (P <0.01); AITP, pancytopenia, neutropenia, thrombocytopenia and hypersplenism were all higher than those in the normal group Difference, P> 0.05; Conclusion NAP on blood disease at different stages, before and after treatment of enzyme activity were significantly different, can be used for early diagnosis and differential diagnosis of blood patients The sensitivity and specificity of diagnostic indicators.
