目的:了解浙江省流动儿童免疫规划服务成本状况,探讨规范开展流动儿童免疫规划工作的合理成本,为争取各级政府对免疫规划工作投入经费提供依据。方法:在浙江省内选择5个县作为流动儿童免疫规划服务成本调查现场,测算当前县、乡、村三级流动儿童免疫规划服务成本,根据调查资料测算村级开展免疫规划工作合理成本,根据有关文献报道的标准测算县、乡级开展免疫规划工作合理成本。结果:浙江省县、乡、村三级流动儿童免疫规划工作平均每针次实际成本为4.49元,县、乡和村每针次服务成本分别为1.09元、3.16元、0.24元;浙江省免疫规划工作平均每针次合理成本为8.85元,县、乡和村免疫规划服务每针次合理成本分别为1.90元、5.60元、1.35元。结论:浙江省免疫规划实际成本以县、乡两级为主,免疫规划实际成本低于合理成本,乡、村级实际成本与合理成本差距较大,提示各级财政应加大对免疫规划经费投入,重点保障乡村级免疫规划工作经费。
OBJECTIVE: To understand the cost status of immunization planning services for migrant children in Zhejiang Province and to explore the reasonable cost of standardizing the immunization planning for migrant children so as to provide evidences for all levels of government to invest in immunization planning. Methods: Five counties in Zhejiang Province were selected as the investigation site for the cost of service for migrant children immunization, and the current cost of immunization planning for migrant children at county, township and village levels was calculated. Based on the survey data, the reasonable cost of immunization planning at village level was calculated. The standard reported in the literature estimates counties and townships to carry out immunization programs at reasonable costs. Results: The average cost per needle for immunization planning of migrant children at county, township and village levels in Zhejiang Province was 4.49 yuan per capita, and the cost per needle for county, township and village clinics was 1.09 yuan, 3.16 yuan and 0.24 yuan respectively. In Zhejiang Province, The average cost per planning time is 8.85 yuan, and the reasonable cost for immunization planning services at county, township and village levels is 1.90 yuan, 5.60 yuan and 1.35 yuan respectively. Conclusion: The actual cost of immunization planning in Zhejiang Province is mainly at county and township levels. The actual cost of immunization programs is lower than the reasonable cost. There is a big gap between the actual costs and the reasonable costs of township and village levels, suggesting that finance for immunization should be increased at all levels Investment, focusing on the protection of village-level immunization program funding.
