目的:明确基于肝脏影像报告与数据系统(liver imaging reporting and data system, LI-RADS)的MRI对肝细胞癌(hepatocellular carcinoma, HCC)中细胞角蛋白19(cytokeratin 19, CK19)表达的诊断价值。方法:回顾性分析复旦大学附属中山医院2017年1月至12月收治的HCC患者临床资料,比较CK19(+)及CK19(-)HCC患者的临床、病理资料及基于2018版LI-RADS的MRI图像特征,进行定性定量分析并通过多因素回归分析确定评估CK19表达的主要预测指标。结果:本研究共纳入78例患者,男性62例,女性16例,年龄(56.1±9.5)岁。甲胎蛋白水平及靶样表现在HCC患者CK19(+)与CK19(-)组间差异均具有统计学意义(均n P0.05)。n 结论:基于LI-RADS的MRI靶样表现结合高甲胎蛋白水平可提示HCC中CK19表达。“,”Objective:To study the diagnostic value of cytokeratin 19 (CK19) expression in hepatocellular carcinoma (HCC) based on MRI liver imaging reports and data system (LI-RADS).Methods:The clinical data of HCC patients who were enrolled in Zhongshan Hospital, Fudan University between January and December 2017 were retrospectively studied. The clinical, pathological and MRI features based on LI-RADS 2018 on patients with CK19-positive and CK19-negative HCCs were analyzed and compared qualitatively and quantitavely. Multivariate logistic regression analyses were performed to identify independent predictors for CK19 expression.Results:A total of 78 patients were included in this study including 62 males and 16 females, with a Mean±SD age of (56.1±9.5) years. The serum alpha-fetoprotein (AFP) levels and targetoid appearance were different between the patients with CK19-positive and CK19-negative HCCs (n P0.05).n Conclusion:Targetoid appearance based on LI-RADS combined with elevated serum AFP levels indicated a positive CK19 expression in HCC.
