目的:研究原发性慢性闭角型青光眼(primary chronic angle-closure glaucoma,PCACG)发病机制中晶体因素的影响。方法:采用A/B超、超声生物显微镜等对原发性慢性闭角型青光眼25例42眼及正常眼24例45眼的晶体参数进行测量和计算。比较组间晶体参数的差异。结果:原发性慢性闭角型青光眼与正常组比较,晶体中央及周边部厚度、前、后表面曲率半径、相对位置、晶体体积、虹膜晶体接触距离,有统计学差异(P<0.05);PCACG组的晶体直径与正常组比较,无统计学差异。且随年龄增加,前房深度、晶体体积、晶体前、后表面曲率均有增加。结论:晶体因素导致的瞳孔阻滞是原发性慢性闭角型青光眼发病的主要诱因,包括晶体较厚,前、后表面曲率较大和相对位置偏前、虹膜晶体接触距离较大等。
Objective: To study the influence of crystal factors in the pathogenesis of primary chronic angle-closure glaucoma (PCACG). Methods: The crystal parameters of 42 eyes of 25 cases with chronic angle-closure glaucoma and 24 cases of 45 eyes with normal eyes were measured and calculated by A / B ultrasonography and ultrasound biomicroscopy. Differences in crystal parameters between groups were compared. Results: Compared with the normal group, the thickness, the radius of curvature of the anterior and posterior surfaces, relative position, crystal volume and contact distance of iris crystal in the patients with primary chronic angle - closure glaucoma were significantly different (P <0.05). Crystal diameter of PCACG group compared with the normal group, no significant difference. And with age, anterior chamber depth, crystal volume, crystal front and back surface curvature have increased. Conclusion: The pupillary block caused by crystal factors is the main cause of primary chronic angle-closure glaucoma, including thicker crystals, larger curvature of the anterior and posterior surfaces and more pre-iris position and larger iris crystal contact distance.
