目的:探讨3D打印技术在后腹腔镜下保留肾单位手术(nephron sparing surgery,NSS)治疗肾肿瘤中应用价值。方法:选取2014年2月~2016年3月我院收治的肾肿瘤患者66例,根据随机数表法将其分为对照组和观察组,每组33例。对照组单用后腹腔镜下NSS治疗,观察组结合3D打印技术实施后腹腔镜下NSS治疗。对两组病理学检查、手术时间、肾蒂阻断时间、胃肠道恢复时间、住院时间等手术指标及术后并发症展开比较。随访两组术后复发情况。结果:两组手术病理学检查比较,差异无统计学意义(P>0.05);观察组手术时间、肾蒂阻断时间各为(96±4)min、(26±2)min,较对照组的(118±6)min、(30±2)min均明显缩短,差异有统计学意义(P<0.05);观察组术中出血量、胃肠道恢复时间等指标与对照组比较,差异无统计学意义(P>0.05);两组并发症发生率比较,差异无统计学意义(P>0.05);两组术后均未复发。结论:3D打印技术在后腹腔镜下NSS治疗肾肿瘤中应用价值显著,可缩短手术时间、肾蒂阻断时间,减低手术风险,值得应用。
Objective: To explore the value of 3D printing in the treatment of renal tumors with retroperitoneal nephron sparing surgery (NSS). Methods: Sixty-six patients with renal tumor admitted to our hospital from February 2014 to March 2016 were divided into control group and observation group according to random number table method, with 33 cases in each group. The control group was treated with retroperitoneal laparoscopic NSS only. The observation group was treated with laparoscopic NSS with 3D printing technique. Two groups of pathological examination, operation time, renal pedicle block time, gastrointestinal recovery time, hospital stay and other surgical indicators and postoperative complications were compared. Follow-up two groups of postoperative recurrence. Results: There was no significant difference between the two groups in the pathological examination (P> 0.05). The operative time and the renal pedicle block time in the observation group were (96 ± 4) min and (26 ± 2) min respectively, (118 ± 6) min, (30 ± 2) min were significantly shorter, the difference was statistically significant (P <0.05); the observation group intraoperative blood loss, gastrointestinal recovery time and other indicators compared with the control group, the difference was Statistical significance (P> 0.05). There was no significant difference in the incidence of complications between the two groups (P> 0.05). There was no recurrence after operation in both groups. Conclusion: The application of 3D printing technique in retroperitoneal NSS in the treatment of renal tumors is significant. It can shorten the operation time, the time of renal pedicle block, and reduce the risk of surgery, which is worthy of application.
