目的探讨不同乙醇摄入量的心肌梗死患者冠状动脉侧支循环水平及血清中血管内皮生长因子(VEGF)水平的变化。方法收集经冠状动脉造影证实至少存在一支主要血管闭塞的冠心病患者103例,对患者侧支循环水平进行Rentrop分级;采用问卷调查收集患者的饮酒量、吸烟、高血压、糖尿病、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)等冠心病危险因素;并根据患者不同饮酒量分为非饮酒组、少量饮酒组(每日乙醇摄入量≤40 g)和大量饮酒组(每日乙醇摄入量>60 g);收集患者动脉血,采用ELISA法检测血清VEGF水平。结果少量饮酒组侧支循环水平高于非饮酒组(P<0.05)和大量饮酒组(P<0.01);少量饮酒组血清VEGF水平(148.32±60.80)pg/mL明显高于非饮酒组(113.53±61.82)pg/mL(P<0.05),而大量饮酒组与少量饮酒组血清VEGF水平无显著性差异(P>0.05)。结论少量饮酒有利于心肌梗死患者冠状动脉侧支循环的建立,可能与血中VEGF高表达有关。
Objective To investigate the changes of coronary collateral circulation and serum levels of vascular endothelial growth factor (VEGF) in patients with myocardial infarction with different ethanol intakes. Methods A total of 103 coronary artery disease patients with at least one major vascular occlusion were confirmed by coronary angiography. Rentrop classification was performed on the collateral circulation level. Alcohol consumption, smoking, hypertension, diabetes mellitus, high density lipids (HDL) and low density lipoprotein (LDL). According to the patients’ different alcohol consumption, they were divided into non-drinking group, a small amount of drinking group (daily ethanol intake ≤40 g) and a large amount of drinking group Daily ethanol intake> 60 g); collected from patients with arterial blood, serum VEGF levels were detected by ELISA. Results The levels of collateral circulation in a small amount of drinking group were significantly higher than those in non-drinking group (P <0.05) and heavy drinking group (P <0.01). The serum levels of VEGF in a small amount of drinking group (148.32 ± 60.80) pg / mL were significantly higher than those in non-drinking group ± 61.82) pg / mL (P <0.05). There was no significant difference in serum VEGF levels between a large number of drinking groups and a small amount of drinking groups (P> 0.05). Conclusion A small amount of alcohol is beneficial to the establishment of coronary collateral circulation in patients with myocardial infarction, which may be related to the high expression of VEGF in blood.
