目的:研究64排螺旋CT血管造影在颅内动脉瘤诊断中的应用。材料与方法:我院2012年1月-2014年1月收治了112例颅内动脉瘤患者,选取其中依据64排螺旋CTA结果进行手术治疗的患者96例。64排螺旋CTA检查:扫描层厚0.625 mm,后处理技术包括应用多层面法(multip lane reconstruction,MPR)、容积显示法(volume rendering,VR)和最大密度投影法(maximum intensity projection,MIP)进行三维图像重建。并对照手术结果评价64排螺旋CTA在颅内动脉瘤诊断中的应用价值。结果:64排螺旋CTA共发现96例104个动脉瘤,其中88例为单发动脉瘤,8例为2个动脉瘤。动脉瘤瘤体最小直径1.9 mm,最大直径37.4 mm,平均15.3 mm。所有患者均依据64排螺旋CTA的结果及SSD模拟的手术入路影像完成手术,在术中均能顺利找到并夹闭动脉瘤,共夹闭动脉瘤104个。CTA能清晰显示动脉瘤大小、位置、形态及与周围结构的关系。手术中发现动脉瘤所在的位置与CTA检查结果一致,符合率达到100%。结论:64排螺旋CTA对颅内动脉瘤的诊断有较高的准确性,对制定动脉瘤的治疗方案有重要的指导价值,适合在临床应用中得到推广。
Objective: To study the application of 64-slice spiral CT angiography in the diagnosis of intracranial aneurysms. Materials and Methods: A total of 112 patients with intracranial aneurysms were enrolled from January 2012 to January 2014 in our hospital. 96 patients were selected according to 64-slice spiral CTA. 64-slice spiral CTA examination: The scan thickness was 0.625 mm. The post-processing techniques included the use of multip lane reconstruction (MPR), volume rendering (VR) and maximum intensity projection (MIP) Three-dimensional image reconstruction. And evaluate the application of 64-slice spiral CTA in the diagnosis of intracranial aneurysms. Results: A total of 96 aneurysms of 96 cases were found in 64 rows of spiral CTA, including 88 aneurysms and 8 aneurysms. The smallest aneurysm tumor diameter of 1.9 mm, the maximum diameter of 37.4 mm, an average of 15.3 mm. All patients underwent surgery based on the results of 64-slice spiral CTA and SSD simulated surgeries. The aneurysms were successfully found and occluded during operation, and 104 aneurysms were closed. CTA can clearly show the aneurysm size, location, morphology and the relationship with the surrounding structure. The location of the aneurysm found in the surgery was consistent with that of the CTA examination, and the coincidence rate reached 100%. Conclusion: The 64-slice spiral CTA has a high accuracy in the diagnosis of intracranial aneurysms, which has important guiding value for the treatment of aneurysms and is suitable for clinical application.
