目的了解输注冷沉淀(Cryo)对多发伤患者血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1α(IL-1α)、C-反应蛋白(CRP)等炎性介质的影响,以探讨冷沉淀促进多发伤愈合的机制。方法 35例多发伤患者随机分为治疗组(17例)和对照组(18例)。治疗组于伤后当天首次输注冷沉淀10 U,再次输注冷沉淀为每12小时5 U,疗程3 d,于输注冷沉淀治疗后的第1、7天同时检测治疗组和对照组的血清TNF-α、IL-1α、CRP的含量。结果输注沉淀治疗后第1天治疗组血浆Fn含量高于对照组(P<0.01);治疗组输注冷沉淀治疗后的第1、7天患者血清TNF-α、IL-1α、CRP含量均低于同期对照组(P<0.05)。结论输注冷沉淀治疗,可以提升患者血浆纤维结合蛋白(Fn)水平,改善多发伤患者单核-吞噬细胞系统功能,降低血清TNF-α、IL-1α、CRP等的炎性介质水平。
Objective To investigate the effect of Cryo on inflammatory mediators such as serum tumor necrosis factor-α (TNF-α), interleukin-1α (IL-1α) and C-reactive protein (CRP) To explore the mechanism of cryoprecipitation to promote multiple wound healing. Methods 35 cases of multiple trauma patients were randomly divided into treatment group (17 cases) and control group (18 cases). In the treatment group, 10 U of cryoprecipitate was infused for the first time on the day after injury, and the cryoprecipitate was again infused for 5 U every 12 hours for 3 days. Results The level of plasma Fn in the treatment group was significantly higher than that in the control group on the first post-infusion treatment (P <0.01). The levels of TNF-α, IL-1α and CRP in the patients on the first and seventh days after the infusion of cryoprecipitate Were lower than the same period control group (P <0.05). Conclusion The infusion of cryoprecipitate can improve the level of plasma Fibronectin (Fn), improve the function of mononuclear phagocytes in multiple traumatic patients, and decrease the levels of inflammatory mediators such as serum TNF-α, IL-1α and CRP.
