目的探讨晚扎脐带对新生儿血红蛋白和网织红细胞的影响。方法将120例新生儿随机分为观察组和对照组,每组60例。观察组新生儿分娩后2 min后进行脐带结扎,对照组新生儿分娩后即进行脐带结扎,观察两组新生儿出生后4周血红蛋白、网织红细胞和胆红素水平。结果两组新生儿血红蛋白水平出生后开始下降,但对照组下降水平更为显著,在各个时间对照组血红蛋白水平均低于观察组,差异有统计学意义(P<0.05),第2周两组网织红细胞水平均显著增高,第3和第4周开始下降,并且观察组网织红细胞水平显著高于对照组,差异有统计学意义(P<0.01),两组胆红素水平在各时间段差异均无统计学意义(P>0.05)。结论晚扎脐带比常规断脐更加有利于增加血红蛋白和网织红细胞的含量,并且不会增加新生儿胆红素水平,值得临床推广应用。
Objective To investigate the effect of umbilical cord on neonatal hemoglobin and reticulocyte. Methods 120 newborns were randomly divided into observation group and control group, 60 cases in each group. The newborn infants in the observation group were ligated 2 minutes after delivery and umbilical cord ligature was performed in the control group after delivery. The hemoglobin, reticulocyte and bilirubin were observed at 4 weeks after birth in the two groups. Results The level of hemoglobin in newborns of the two groups began to decline after birth, but the level of hemoglobin in the control group was more significant. The levels of hemoglobin in the control group were lower than those in the observation group at each time point, with significant difference (P <0.05) Reticulocyte levels were significantly increased, the first 3 weeks and 4 weeks began to decline, and the observation group reticulocyte levels were significantly higher than the control group, the difference was statistically significant (P <0.01), two groups of bilirubin levels at each time There was no significant difference between the segments (P> 0.05). Conclusion The umbilical cord ligament at night is more conducive to increase the hemoglobin and reticulocyte content than the normal umbilical cord, and will not increase the bilirubin level in neonates, which is worthy of clinical application.
