目的 :探讨原发性干燥综合征(pSS)患者外周血B细胞瘦素(LP)及瘦素受体(LPR)的表达及临床意义。方法:应用流式细胞仪(FCM)检测56例pSS患者及19例正常对照者外周血B细胞LP及LPR的表达,分析LP及LPR表达与患者临床特点及疾病活动的相关性。结果:pSS患者B细胞LP表达与正常对照组差异无统计学意义(P>0.05),而LPR表达高于正常对照组,差异有统计学意义(P=0.029)。活动期患者B细胞LP表达与稳定期差异无统计学意义(P>0.05),而LPR表达高于稳定期,差异有统计学意义(P=0.037)。患者B细胞LP表达与LPR表达呈正相关,具有统计学意义(r=0.332,P=0.004)。患者B细胞LP表达与低密度脂蛋白(r=-0.276,P=0.030)、肌酐(r=-0.256,P=0.034)呈负相关,具有统计学意义。患者B细胞LPR表达与体重(r=-0.294,P=0.010)、BMI(r=-0.296,P=0.010)呈负相关,具有统计学意义。口腔溃疡组B细胞LPR表达高于无口腔溃疡组,差异有统计学意义(P<0.01)。结论:pSS患者B细胞LPR存在异常,与病情活动相关,B细胞上LP与LPR显著正相关,可以调节脂肪代谢及体重,可能参与到pSS疾病中肾脏损伤及口腔溃疡。
Objective: To investigate the expression of leptin (LP) and leptin receptor (LPR) in peripheral blood mononuclear cells of patients with primary Sjogren’s Syndrome (pSS) and its clinical significance. Methods: The expression of LP and LPR in peripheral blood mononuclear cells of 56 patients with pSS and 19 normal controls were detected by flow cytometry (FCM). The correlation between the expression of LP and LPR and the clinical features and disease activities were analyzed. Results: The expression of LP in B cells of pSS patients was not significantly different from that of normal controls (P> 0.05), but the expression of LPR was higher than that of normal controls (P = 0.029). There was no significant difference in LP expression and stable phase among patients with active stage (P> 0.05), while LPR expression was higher than that in stable stage (P = 0.037). The expression of LP in patients with B cells was positively correlated with LPR expression (r = 0.332, P = 0.004). The expression of LP in B cells of patients was negatively correlated with LDL (r = -0.276, P = 0.030) and creatinine (r = -0.256, P = 0.034) The expression of LPR in patients with B cells was negatively correlated with body weight (r = -0.294, P = 0.010) and BMI (r = -0.296, P = 0.010) LPR expression in oral ulcer group was higher than that in oral ulcer group, the difference was statistically significant (P <0.01). CONCLUSIONS: There is abnormal LPR in B cells of patients with pSS, which is related to disease activity. There is a significant positive correlation between LP and LPR on B cells, which can regulate fat metabolism and body weight, which may be involved in renal injury and oral ulcer in pSS.
