目的:通过比较仪器法和手工法计数单采血小板中混入的红细胞和残留的白细胞,评价2种方法在单采血小板的质量评价过程中的适用性。方法:收集MCS+机型、Amicus机型和Trima机型3种仪器设备采集的保存期内的单采血小板各30例,记录献血员的基本资料,用全自动血细胞计数仪法和手工法计数单采血小板中混入的红细胞和白细胞,然后进行统计学分析。结果:献血前3组机型的献血员HCT、外周血WBC、RBC均差异无统计学意义。3组机型采集的单采血小板产品每袋血小板含量差异无统计学意义(P>0.05)。3组机型的红细胞混入量仪器法计数均高于手工法(配对t检验,P<0.05),手工法计数可显著提高红细胞混入量的合格率(χ2检验,P<0.05)。MCS+机型的单采血小板中白细胞混入量手工法和仪器法计数差异无统计学意义(P>0.05),Amicus和Trima 2种机型的单采血小板产品手工法计数白细胞均低于仪器法(P<0.05),但手工计数法只能提高Amicus机型的白细胞混入量合格率,对另外2种机型的单采血小板白细胞混入量合格率无影响。结论:在对单采血小板进行质量评价时,对于红细胞混入量的计数,手工计数比全自动血细胞计数仪更适用;但对于白细胞混入量的计数,MCS+机型和Trima机型2种方法均可,而Amicus机型手工法比全自动血细胞计数仪更适用。
OBJECTIVE: To evaluate the applicability of the two methods in assessing the quality of apheresis platelets by counting the erythrocytes and residual leukocytes mixed in the apheresis plate by comparing the instrumental method and manual method. Methods: 30 samples of apheresis platelets collected during the storage period of three kinds of instruments and equipment collected by MCS +, Amicus and Trima models were collected. The basic data of blood donors were collected and analyzed by automatic hemocytometer and manual counting Blood samples were mixed with red blood cells and white blood cells, and then for statistical analysis. Results: There was no significant difference in HCT, WBC and RBC among the three groups before blood donation. There was no significant difference in the platelet counts per bag of the apheresis platelets collected from the three models (P> 0.05). The count of erythrocyte mixed volume was higher than that of the manual method (paired t test, P <0.05). The counting by manual method could significantly improve the qualified rate of mixed red blood cells (χ2 test, P <0.05). There was no significant difference between the manual method and the instrumental method (P> 0.05) for the amount of leucocyte mixed in the apheresis platelets of the MCS + model. The leukocyte count of the apheresis platelets of both the Amicus and Trima models was lower than that of the instrumental method P <0.05), but manual counting method can only improve the qualified rate of leukocyte incorporation in Amicus model, and has no effect on the pass rate of leukocyte incorporation in the other two models. CONCLUSIONS: When evaluating the quality of apheresis platelets, manual counting is more suitable than automatic hemocytometer for counting the number of erythrocytes. However, both the MCS + model and the Trima model can be used for counting the amount of white blood cells mixed , While the Amicus model manual method is more applicable than automatic hemocytometer.
