目的:分析燃煤污染型地氟病区中学生心理疾病患病情况,探讨该地区中学生心理健康现状的影响因素,为制定心理疏导对策提供依据。方法:采用分层整群随机抽样方法对古蔺县1839名初中生和高中生进行中国中学生心理健康量表(MMHI-60)问卷调查和氟斑牙诊断。结果:检出有心理问题的中学生占调查人数的41%,检出有轻度、中度、重度心理问题的各占36.9%、4.0%、0.1%。女生、年级高、对氟斑牙态度为负向、氟斑牙自我判定为“有”是中学生发生心理问题的危险因素,OR值(95%CI)分别是1.340(1.048-1.714)、1.324(1.196-1.465)、3.010(1.202-7.536)、1.623(1.086-2.462);成绩好、家庭经济富裕是保护因素,OR值(95%CI)为0.757(0.640-0.897)和0.646(0.493-0.846)。结论:地氟病区中学生心理问题不容乐观,性别、年级、氟斑牙自我判断、对氟斑牙的态度、成绩、及家庭经济状况与中学生心理健康存在相关性,应针对相关因素采用综合防治措施,以促进该地区中学生心理健康发展。
Objective: To analyze the prevalence of mental illness among middle school students in coal-contaminated endemic fluorosis area and to explore the influencing factors of mental health status of middle school students in this area, and to provide basis for making psychological counseling strategies. Methods: A stratified cluster random sampling method was used to survey the middle school students’ mental health scale (MMHI-60) questionnaire and dental fluorosis in 1839 middle school students and high school students in Gulin County. Results: There were 41% of high school students who detected psychological problems and 36.9%, 4.0% and 0.1% of them were mild, moderate and severe psychological problems respectively. The female students, whose grade was high, had a negative attitude toward dental fluorosis, and their self-judgment was “” with high school students. The odds ratio (95% CI) was 1.340 (1.048-1.714) 1.324 (1.196-1.465), 3.010 (1.202-7.536), 1.623 (1.086-2.462). The scores were good and family economic affluence was the protective factor. The OR values (95% CI) were 0.757 (0.640-0.897) and 0.646 0.846). Conclusion: The psychological problems of middle school students in the endemic fluorosis area are not optimistic. There is a correlation between the attitude of secondary school students, sex, grade, dental fluorosis, their attitude toward dental fluorosis, their family financial status and the mental health of middle school students.
