目的探讨用尿道刀及钬激光腔内治疗尿道狭窄的临床效果。方法43例尿道狭窄的患者用美国Circom公司生产的尿道内切开镜、尿道刀及美国科医人公司生产的LUMENISPOWERSuite100W钬激光行尿道内切开术治疗。结果所有的患者均一次手术治愈,术后最大尿流率超过18mL/s,其中18例经钬激光内切开、25例经尿道刀内切开,术后3个月内常规行尿道扩张术。狭窄部位:前尿道28例(包括1例前尿道全段狭窄及1例尿道闭锁患者)、膜部尿道7例、膀胱颈部尿道8例。狭窄长度:除1例前尿道全段狭窄外,其他狭窄长度在0.3 ̄3.0cm之间。结论钬激光及Sachse刀均为腔内治疗尿道狭窄的有效方法,但对于长段的尿道狭窄、尿道闭锁及合并结石者,钬激光优于尿道刀。
Objective To investigate the clinical effects of urethral knife and holmium laser in the treatment of urethral stricture. Methods Forty-three patients with urethral stricture were treated with endoscopic urethrotomy, urethral knife and LUMENISPOWERSuite100W holmium laser produced by Circom Company of America. Results All the patients were surgically cured and the maximum postoperative uroflow rate was over 18 mL / s. Among them, 18 patients underwent holmium laser incision and 25 patients underwent transurethral resection of the urethra. Conventional urethral dilation was performed within 3 months after operation. . Stenosis: 28 cases of anterior urethra (including 1 case of anterior urethral stricture and 1 case of urethral atresia), 7 cases of membranous urethra and 8 cases of bladder neck and urethra. Narrow length: In addition to a case of anterior urethral stricture, other stenosis length between 0.3 ~ 3.0cm. Conclusions Both holmium laser and Sachse knife are effective methods for the treatment of urethral stricture. However, holmium laser is superior to urethral knife for long urethral stricture, urethral atresia and complicated stone.
