目的研究纤维母细胞活化蛋白(FAP)和转化生长因子β1(TGFβ1)在胃癌组织中的表达,并分析其与胃癌临床病理特征及淋巴结转移间的关系。方法采用免疫组化方法检测FAP和TGFβ1在42例胃癌和癌旁正常组织中的表达情况。结果①正常胃组织中FAP(-);胃癌组织中FAP主要表达于肿瘤相关纤维母细胞胞质中,且弥漫型胃癌表达显著高于肠型胃癌(P<0.05),并与临床病理分期、浸润深度和淋巴结转移呈正相关(P<0.05);②正常胃组织中TGFβ1为(-)或弱(+);胃癌组织中TGFβ1主要表达于瘤细胞胞质,在肿瘤间质纤维母细胞、血管内皮细胞和中性粒细胞偶有表达,且瘤细胞的表达强度与临床病理分期、浸润深度和淋巴结转移呈正相关(P<0.05);③FAP在肿瘤相关纤维母细胞的表达与TGFβ1在肿瘤细胞的表达呈正相关(P<0.05)。结论从FAP表达与TGFβ1表达的正相关性,推测其可能是通过肿瘤-间质相互作用促进肿瘤演进。FAP特异性表达于胃癌间质纤维母细胞,与多项临床病理指标,尤其是淋巴结转移和肿瘤的病理分型有关,可以作为预测胃癌淋巴结转移和预后的免疫病理学指标,有望成为抗胃癌治疗的一个新的靶向分子。
Objective To study the expression of fibroblast activating protein (FAP) and transforming growth factor-β1 (TGFβ1) in gastric carcinoma and to analyze its relationship with clinicopathological features and lymph node metastasis. Methods Immunohistochemistry was used to detect the expression of FAP and TGFβ1 in 42 cases of gastric cancer and adjacent normal tissues. Results FAP (-) in normal gastric tissues was mainly expressed in the cytoplasm of tumor-associated fibroblasts, and was significantly higher in diffuse gastric carcinoma than that in intestinal type gastric carcinoma (P <0.05) (P <0.05); ② TGFβ1 was (-) or weakly (+) in normal gastric tissue; TGFβ1 was mainly expressed in the cytoplasm of tumor cells in gastric cancer tissue, and there was significant difference in the expression of TGFβ1 in tumor interstitial fibroblasts, The expressions of endothelial cells and neutrophils were occasionally expressed, and the expression intensity of tumor cells was positively correlated with the clinicopathologic stage, depth of invasion and lymph node metastasis (P <0.05). ③ The expression of FAP in tumor-associated fibroblasts was positively correlated with the expression of TGFβ1 in tumor cells The expression was positively correlated (P <0.05). Conclusion From the positive correlation between FAP expression and TGFβ1 expression, we speculate that it may promote the tumor evolution through tumor-interstitial interaction. FAP specific expression in gastric stromal fibroblasts, and a number of clinical and pathological indicators, especially lymph node metastasis and tumor pathological typing, can be used as a prediction of gastric cancer lymph node metastasis and prognosis of the immunopathological indicators, is expected to become anti-gastric cancer treatment A new target molecule.
