[目的]探讨侧脑室肿瘤的手术入路选择及显微外科治疗技术。[方法]回顾性分析我院自2000年6月~2009年6月显微手术治疗的28例侧脑室肿瘤的临床资料,详细分析不同部位肿瘤的手术入路及显微切除技术。其中经额中回皮质入路9例,经纵裂胼胝体入路4例,经顶小叶皮层入路2例,经颞中回皮层入路4例,经顶枕叶皮层入路9例。[结果]肿瘤全切除21例,次全切除5例,大部分切除2例,本组无手术死亡病例。术后出现脑积水5例,肢体轻瘫3例,偏盲1例,癫痫2例;3例星形细胞瘤复发,其中1例死亡;1例室管膜瘤3级,术后2.5年全脑室复发死亡。[结论]选择最佳的手术入路,运用显微神经外科技术治疗侧脑室肿瘤可提高肿瘤全切率,减少并发症。
[Objective] To investigate the surgical approach and microsurgical treatment of lateral ventricle tumors. [Methods] The clinical data of 28 cases of lateral ventricle tumors treated by microsurgery in our hospital from June 2000 to June 2009 were retrospectively analyzed. The surgical approaches and microdissection techniques were analyzed in detail. Among them, there were 9 cases with recanalization of cortex, 4 cases by longitudinal incision of corpus callosum, 2 cases by transplanted corpus callosum, 4 cases by transcortical approach, 9 cases by transoccipital cortex. [Results] Totally resected tumors in 21 cases, subtotal resection in 5 cases, the majority of resection in 2 cases, no surgical deaths in this group. Postoperative hydrocephalus in 5 cases, 3 cases of limb palsy, hemianopia in 1 case, 2 cases of epilepsy; 3 cases of astrocytoma recurrence, 1 case died; 1 case of ependymoma 3, 2.5 years after surgery Ventricular recurrence and death. [Conclusion] Choosing the best surgical approach, the use of microsurgical neurosurgery for the treatment of lateral ventricle tumors can improve the rate of tumor resection and reduce complications.
