应用多聚酶键反应—聚丙烯酰胺凝胶电泳(PCR-PAGE)和银染色法对164例恶性血液病患者进行了免疫球蛋白重链基因(IgH)、T细胞受体γ链基因(TCRγ)的克隆性重排检测。结果显示,重排之IgH与TCRγ总比例为3∶1,ALL进展期IgH占50%,TCRγ占27.3%,双阳性18.2%,总计阳性率为95.5%。当骨髓象缓解时克隆重排仍占60%,MM有85%为IgH重排,5%为TCRγ,缓解时仍有25%呈IgH重排。TCRγ及双阳性所占比例上升与病情恶化呈正相关。因此以这种检测方法来指导化疗可以提高生存率和治愈率。
Polyclonal enzyme reaction-polyacrylamide gel electrophoresis (PCR-PAGE) and silver staining were performed on 164 patients with hematological malignancies who had immunoglobulin heavy chain gene (IgH) and T cell receptor gamma chain gene (TCRγ) Clonal rearrangement detection. The results showed that the total proportion of IgH and TCRγ in the rearrangement was 3:1, IgH in advanced ALL stage was 50%, TCRγ was 27.3%, double positive was 18.2%, and the total positive rate was 95.5%. When the bone marrow was relieved, the clone rearrangement still accounted for 60%, 85% of the MM were IgH rearrangements, 5% were TCRγ, and 25% showed IgH rearrangement when remission. The increase in the proportion of TCRγ and double positive was positively correlated with the worsening of the disease. Therefore, the use of this test to guide chemotherapy can improve survival and cure rates.
