[目的]探讨四川省盐亭县的食道癌发病影响因素,为该地区的食道癌预防工作提供依据。[方法]在盐亭县40~69岁的4038名调查对象中,将食道碘染色内镜检查正常者作为对照,病理活检结果为重增/原位癌、黏膜内癌及浸润癌作为病例,进行面对面问卷调查,并运用非条件Logistic回归,分析食道癌发病影响因素。[结果]食道癌发病危险因素有:吸烟(OR=2.250,OR95%CI:1.386~3.651)、配偶患消化系统其他肿瘤(OR=13.414,OR95%CI:1.168~154.011)、一级亲属患食道癌(OR=2.583,OR95%CI:1.240~5.382)、一级亲属患胃癌(OR=3.533,OR95%CI:1.399~8.918)。[结论]四川盐亭的食道癌高发不仅与饮食因素、生活习惯有关,还与食道癌、胃癌等消化系统肿瘤家族史有关。
[Objective] To investigate the influencing factors of esophageal cancer in Yanting County, Sichuan Province, and provide the basis for prevention of esophageal cancer in this area. [Methods] Among 4038 surveyed 40- to 69-year-olds in Yanting County, normal endoscopic iodine staining was used as a control, pathological biopsy results were ascending / carcinoma in situ, intramucosal carcinoma and invasive carcinoma as cases, Face-to-face questionnaire was conducted, and non-conditional logistic regression was used to analyze the influencing factors of esophageal cancer. [Results] The risk factors for esophageal cancer were smoking (OR = 2.250, OR95% CI: 1.386-3.651), other spouses with other digestive system tumors (OR = 13.414, OR95% CI: 1.168-154.011) Cancer (OR = 2.583, OR95% CI: 1.240-5.382), and first degree relatives had gastric cancer (OR = 3.533, OR95% CI: 1.399-8.918). [Conclusion] The high incidence of esophageal cancer in Yanting, Sichuan Province is not only related to dietary factors and lifestyle, but also to the family history of digestive system tumors such as esophagus and stomach cancer.
