目的调查当地某医院门诊处方书写不规范和不合理用药情况,提高处方质量,促进临床合理用药,减少不良反应的发生。方法对门诊处方18491张,逐一分析处方是否规范、合理,并对不合格的处方进行归类评价。结果书写不规范处方6448张,不合格率为34.9%,不合理用药处方1208张,不合格率为6.5%,剩余10835张处方均符合规范处方及合理用药处方。结论该医院存在一定程度的处方不合格现象,主要体现在处方书写错误、不合理用药形式多样,应引起临床医师和药师的重视。
Objective To investigate the nonstandard and irrational prescriptions of outpatient prescriptions in a local hospital, improve the quality of prescriptions, promote the rational use of clinical medicine and reduce the occurrence of adverse reactions. Methods 18491 outpatient prescriptions were analyzed one by one to determine whether the prescriptions were standardized and reasonable and the unqualified prescriptions were classified and evaluated. The results of non-standard prescription writing 6448, the failure rate was 34.9%, 1208 unreasonable drug prescriptions, the unqualified rate was 6.5%, the remaining 10,835 prescriptions are in line with the standard prescription and rational drug prescriptions. Conclusion There is a certain degree of prescription failure in the hospital, which is mainly reflected in the fact that prescription writing errors and irrational dosage forms should be taken seriously by clinicians and pharmacists.
