本文报告160例顽固性癫痫手术治疗中脑皮质电极的应用情况,其中胼胝体前2/3切开加额叶切除20例,术前探查均为广泛爆发棘慢波,术后恢复正常17例,残余单个尖波3例;大脑凸面功能区痫灶多软膜下横切术84例(左40例,右44例),79例术后棘波消失,5例明显减少;颞叶切除56例(左30例,右26例),其中20例行单纯前颞叶及海马杏仁核切除,36例在切除后对颞叶新皮质痫灶行多软膜下横切术,术后棘波灶全部消失,术后随访6~60个月,癫痫发作消失110例(68.7%),减少50%以上82例(20%);由大发作转为小发作11例(7%),无变化7例(4.8%),总有效率为95.7%,结果表明皮层电极对于术中确定痫灶范围,指导手术治疗极有价值.
This article reports 160 cases of intractable epilepsy surgery in the application of cortical electrodes, including the corpus callosum before 2/3 cut plus frontal resection in 20 cases, preoperative exploration were widely spikes and waves, returned to normal in 17 cases, Residual singular spikes in 3 cases; cerebrum convex functional area epileptic foci of 84 cases (left 40 cases, right 44 cases), 79 cases of spikes disappeared after operation, 5 cases significantly reduced; 56 cases of temporal lobe resection (Left 30 cases, right 26 cases), of which 20 cases of simple anterior temporal and hippocampal amygdala resection, 36 cases after resection of temporal lobe neocortex epilepsy line underwent subretinal transthoracic surgery, postoperative spikes (68.7%), 82 (20%) reduced more than 50%; 11 cases (7%) changed from major to minor attack, no change 7 Cases (4.8%), the total effective rate was 95.7%, the results show that the cortical electrode for the determination of the scope of epileptic foci, instructive surgical treatment of great value.
