目的:探讨乙型肝炎病毒(HBV)携带者血清病毒核酸(HBV-DNA)载量与丙氨酸转移酶(ALT)水平、HBV血清标志物含量的相关性。方法:收集2015年1月至2015年12月清远市人民医院乙肝表面抗原(HBsAg)阳性标本269例,非乙型肝炎健康体检标本31例为对照组。(1)以e抗原为区组因素,分别研究e抗原阳性和e抗原阴性人群中血清谷丙转氨酶(ALT)与HBV-DNA载量的关系;(2)以血清ALT水平为区组因素,研究在排除ALT影响后血清HBV-DNA载量与血清标志物的关系。(3)用直线相关分析HBV-DNA载量与HBsAg、乙肝e抗原(HBeAg)、乙肝核心抗体(HBc Ab)水平的相关性。结果:ALT水平正常的人群中,HBsAg阳性患者的血清ALT水平高于对照组,差异具有统计学意义(P<0.05)。以e抗原为区组因素,同一区组内,ALT升高组血清HBV-DNA载量高于ALT正常组,差异具有统计学意义(P<0.05);e抗原阴性组HBV-DNA载量与血清ALT水平具有线性趋势关系差异具有统计学意义(P<0.05),但e抗原阳性组HBV-DNA载量与血清ALT水平不具有等级相关关系,差异无统计学意义(P>0.05)。HBV-DNA载量与HBsAg、HBeAg相关(P<0.05),HBV DNA载量与HBcAb不具有等级相关关系。结论:(1)HBV感染影响血清ALT水平;(2)同一感染模式中,ALT升高者HBV-DNA载量高于ALT正常者;(3)HBV携带者血清HBV-DNA载量与其血清ALT值、HBeAg、HBsAg值相关,其关联度为HBeAg>HBsAg>ALT,与HBc Ab关联性不大。
Objective: To investigate the relationship between serum HBV (HBV) DNA load and ALT and serum HBV markers in patients with hepatitis B virus (HBV). Methods: From January 2015 to December 2015, 269 HBsAg positive specimens from Qingyuan Municipal People’s Hospital and 31 healthy non-hepatitis B specimens were collected as control group. (1) To study the relationship between serum alanine aminotransferase (ALT) and HBV-DNA load in patients with e antigen positive and e antigen negative population, using e antigen as the grouping factor; (2) Taking the serum ALT level as the grouping factor, To investigate the relationship between serum HBV-DNA load and serum markers after excluding the influence of ALT. (3) The correlation between HBV DNA load and the levels of HBsAg, HBeAg and HBc Ab was analyzed by linear correlation. Results: The serum level of ALT in HBsAg positive patients with normal ALT level was significantly higher than that in control group (P <0.05). Taking e antigen as the group factor, the serum HBV-DNA load in the ALT elevated group was higher than that in the normal ALT group (P <0.05). The HBV-DNA load in the negative e antigen group was Serum ALT levels had a trend of linear trend with statistical significance (P <0.05), but the e antigen positive group HBV-DNA load and serum ALT levels do not have a hierarchical correlation, the difference was not statistically significant (P> 0.05). The HBV DNA load was associated with HBsAg and HBeAg (P <0.05), and the HBV DNA load was not related to HBcAb. (2) In the same infection pattern, the HBV-DNA load of patients with elevated ALT was higher than that of patients with normal ALT; (3) The serum HBV-DNA levels of HBV carriers were significantly correlated with serum ALT Value, HBeAg, HBsAg value, the correlation between the HBeAg> HBsAg> ALT, and HBc Ab correlation is not.
