目的通过分析MRI常规扫描和增强扫描两种不同方法在膝关节损伤诊断中的临床效果对比,为优化膝关节损伤诊断提供一定理论依据。方法选取该院骨关节科2014年5月—2017年5月收治的120例膝关节损伤患者为研究对象,对其进行常规扫描,若发现关节内存在一定的积血,可进行关节穿刺操作,抽出积血,注入增强剂后,进行扫描。若患者关节内未发现积血或者积液,可直接进行注入增强剂,进行增强扫描。结果患者经过常规扫描检查后,发现半月板、软骨、前后交叉韧带、内外侧副韧带均显示有不同程度的损伤。注入增强剂进行扫描后,膝关节损伤诊断的精确度更高。进行常规扫描检查结果发现:39例患者半月板损伤,33例患者内外侧副韧带损伤,24例患者关节内软骨损伤,36例交叉韧带损伤;增强扫描检查结果发现:57例患者半月板损伤,51例患者内外侧副韧带损伤,42例患者关节软骨损伤,两组患者差异有统计学意义(P<0.05)。结论磁共振膝关节内增强扫描与常规MRI扫描相比,灵敏性和特异性较高,具有临床推广的意义。
Objective To analyze the clinical effect of routine MRI and enhanced scan in the diagnosis of knee joint injury and to provide a theoretical basis for the diagnosis of knee injury. Methods A total of 120 patients with knee joint injuries treated in our hospital from May 2014 to May 2017 were enrolled in this study. The patients were routinely screened. If there was some hematoma in the joint, Blood is drawn out, injected with enhancer, and scanned. If no evidence of hemorrhage or effusion in the patient’s joint, inject the enhancer directly and perform an enhanced scan. Results After routine scanning examination, the patient found that the meniscus, cartilage, anterior cruciate ligament, medial and lateral collateral ligament showed different degrees of damage. The accuracy of diagnosis of knee injury is even higher after the injection of enhancer for scanning. The results of routine scanning examination showed that there were 39 cases of meniscus injury, 33 cases of medial-lateral collateral ligament injury, 24 cases of articular cartilage injury and 36 cases of cruciate ligament injury. The results of enhanced scanning examination showed that 57 cases had meniscus injury, 51 cases of injury of medial and collateral ligaments, 42 cases of articular cartilage injury, the difference between the two groups was statistically significant (P <0.05). Conclusion Compared with conventional MRI scan, MRI enhanced MRI has higher sensitivity and specificity, which is of clinical significance.
