目的 探讨指示胃癌临床病理学特征和预后的分子学指标。方法 应用生物素 卵白素复合物法 (ABC法 )检测 171例胃癌标本中 p5 3、C erbB 2、表皮生长因子受体 (EGFR)、nm2 3和E 钙粘蛋白 (E cad)的表达。结果 (1)癌组织浸润越深 ,nm2 3和E cad表达越少 (P <0 .0 5 ) ,nm2 3和E cad表达与淋巴结转移和远处转移呈负相关 (P <0 .0 5 ) ,C erbB 2和EGFR表达与远处转移呈正相关 (P <0 .0 5 ) ,p5 3在弥漫型胃癌组织中表达较高 (P <0 .0 5 ) ,且高表达的胃癌组织易发生淋巴结转移 (P <0 .0 5 )。 (2 ) p5 3和EGFR的表达呈正相关 (P <0 .0 5 ) ;C erbB 2和EGFR的表达呈正相关(P <0 .0 5 ) ;nm2 3和E cad的表达呈正相关 (P <0 .0 1)。 (3)Cox模型多因素分析表明 ,p5 3、C erbB 2及EGFR表达的胃癌病人预后差 (P皆 <0 .0 1) ,而E cad表达则病人预后好 (P <0 .0 1)。结论 测定EGFR、E cad、C erbB 2和 p5 3表达 ,对估计胃癌生物学行为和判断预后有较大帮助。C erbB 2和EGFR同步表达对胃癌的进展可能有协同作用。
Objective To explore the molecular indicators that indicate the clinicopathological features and prognosis of gastric cancer. Methods The expression of p53, c-erbB2, epidermal growth factor receptor (EGFR), nm23, and E-cadherin (E cad) in 171 gastric cancer specimens was detected by biotin-albumin complex method (ABC method). Results (1) The deeper the infiltration of cancer tissue, the less expression of nm23 and Ecad (P < 0.05), and the negative correlation between nm23 and Ecad expression and lymph node metastasis and distant metastasis (P <0. ), CerbB 2 and EGFR expression was positively correlated with distant metastasis (P < 0.05), p53 was highly expressed in diffuse gastric carcinoma (P <0.05), and high expression of gastric cancer tissue was easy. Lymph node metastasis occurred (P < 0.05). (2) The expression of p53 and EGFR were positively correlated (P < 0.05). The expression of CerbB 2 and EGFR was positively correlated (P <0.05). The expression of nm23 and Ecad was positively correlated (P <0. 0 .0 1). (3) Multivariate analysis of Cox model showed that the prognosis of gastric cancer patients with p53, C erbB 2 and EGFR expression was poor (P < 0.01), while E cad expression was a good prognosis (P <0. 01). . Conclusion The determination of EGFR, E cad, CerbB 2 and p53 is helpful to estimate the biological behavior of gastric cancer and prognosis. Synchronous expression of C erbB 2 and EGFR may have synergistic effects on the progression of gastric cancer.
