目的:应用左房容积指数(LAVI)与血清N末端脑钠肽原(NT-pro BNP)水平的变化情况评价原发性高血压(EH)患者左心室舒张功能。方法:选择2013年5月-2015年5月在我院接受治疗的原发性高血压患者100例。根据左房舒张功能将患者分为A组(E/A>1,且Em/Am>1)、B组(E/A<1,且Em/Am<1)、C组(E/A>1,且Em/Am<1)及D组(E/A>2,且Em/Am<1),。另选取同期在我院接受体检的健康志愿者100例作为对照组。观察并比较各组LAVI及NT-pro BNP水平,分析LAVI与及NT-pro BNP与原发性高血压患者左心室舒张功能的相关性关系。结果:原发性高血压患者E/A、LAVI及NT-pro BNP水平均显著高于对照组,而Em/Am则低于对照组,差异具有统计学意义(P<0.05);原发性高血压患者中,LAVI及NT-pro BNP水平随E/A升高而递增,随Em/Am升高而递减,其中D组LAVI及NT-pro BNP水平显著高于其他三组,差异均具有统计学意义(P<0.05)。根据Pearson相关性分析可知结果显示,原发性高血压患者LAVI与NT-pro BNP呈正相关关系(P<0.05),但与E/A及Em/Am无明显相关关系(P>0.05)。LAVI的截断值为29.040,NT-pro BNP为2.065时评价左室舒张功能存在异常的敏感度及特异度均较高(P<0.05)。结论:LAVI及NT-pro BNP可较好地评价EH患者左室舒张功能情况,且:LAVI及NT-pro BNP二者之间联系紧密与原发性高血压存在一定的相关性,两者均能较为准确的评价原发性高血压患者的左心室舒张功能,值得在临床给予推广应用。
Objective: To evaluate the left ventricular diastolic function in patients with essential hypertension (EH) by using the changes of left ventricular volume index (LAVI) and serum NT-pro BNP level. Methods: One hundred patients with essential hypertension treated in our hospital from May 2013 to May 2015 were selected. Patients were divided into group A (E / A> 1 and Em / Am> 1) according to left atrial diastolic function. Group B (E / A <1 and Em / Am <1) 1, and Em / Am <1) and D (E / A> 2 and Em / Am <1). Another 100 cases of healthy volunteers who received physical examination in our hospital during the same period were selected as the control group. The levels of LAVI and NT-pro BNP in each group were observed and compared, and the relationship between LAVI and NT-pro BNP and left ventricular diastolic function in patients with essential hypertension was analyzed. Pearson correlation analysis showed that there was a positive correlation between LAVI and NT-pro BNP in patients with essential hypertension (P <0.05), but no significant correlation with E / A and Em / Am (P> 0.05). LAVI cut-off value of 29.040, NT-pro BNP of 2.065 when the evaluation of left ventricular diastolic function abnormalities were higher sensitivity and specificity were higher (P <0.05). Conclusion: LAVI and NT-pro BNP can be used to evaluate left ventricular diastolic function in patients with EH, and there is a certain correlation between LAVI and NT-pro BNP and essential hypertension. Both of them Can more accurately evaluate the left ventricular diastolic function in patients with essential hypertension, it is worth to promote the clinical application.
