目的检测甲胎蛋白(alpha fetoprotein,AFP)阴性与AFP阳性穿刺活检人肝细胞癌(hepatocellular carcinoma,HCC)中CD34及多克隆癌胚抗原(polyclone carcinoembryonic antigen,p CEA)的表达情况,探讨其病理诊断意义。方法收集2013年—2015年恩施土家族苗族自治州中心医院穿刺活检病理确诊为HCC标本54例,依据血清学及免疫组织化学法(免疫组化)检测AFP结果,将患者分为AFP阳性组(n=33)及AFP阴性组(n=21)。应用免疫组化技术,检测54例HCC组织中CD34、p CEA的表达情况。结果 AFP阳性组p CEA的阳性表达率(69.7%)高于AFP阴性组(38.1%),差异有统计学意义(P<0.05);而CD34在AFP阳性组(90.91%)与AFP阴性组(85.71%)中的阳性表达率比较,差异无统计学意义(P>0.05)。结论 AFP与CD34、p CEA联合检测有利于肝穿刺活检中HCC的诊断及其鉴别诊断。
Objective To detect the expression of CD34 and polyclonal carcinoembryonic antigen (p CEA) in alpha fetoprotein (AFP) negative and AFP positive biopsy human hepatocellular carcinoma (HCC) Diagnostic significance. Methods A total of 54 HCC specimens were collected from the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture during the period from 2013 to 2015. AFP was detected by serological and immunohistochemical methods (immunohistochemistry). The patients were divided into AFP positive group (n = 33) and AFP negative group (n = 21). Immunohistochemistry was used to detect the expression of CD34 and p CEA in 54 HCC tissues. Results The positive expression rate of p CEA in AFP positive group (69.7%) was higher than that in AFP negative group (38.1%), the difference was statistically significant (P <0.05); while the expression of CD34 in AFP positive group (90.91%) and AFP negative group 85.71%), the difference was not statistically significant (P> 0.05). Conclusion Combined detection of AFP, CD34 and p CEA is helpful for the diagnosis and differential diagnosis of HCC in liver biopsy.
