本文报告1例患中枢神经系统结节病的女孩,并复习了23例发生在儿童结节病的中枢神经系统表现。患儿,女,13岁,因发烧、不适、关节痛,呕吐、体重下降一个月而入院。入院前数天感到视力模糊。体温38.5℃,脉搏80次/分,呼吸24次/分,血压120/80mmHg,上下肢伸侧可见明显的色素沉着。巩膜充血、畏光。裂隙灯检查可见明显肉芽肿性全葡萄膜炎。心肺无异常,肝大右肋缘下4厘米.白细胞8800,杆状核30%,中性34%,淋巴26%,嗜酸细胞7%。血沉64mm/L 小时,CPK 179 U/L,尿蛋白(+),镜检白细胞3—5个/高倍。肺功能试验明显降低,肺活量为正常人的30%。结膜或肝活检为阴性。临床诊断为结节病,开始用强地松40mg/日,出院治疗。在家持续恶心、厌食、体重下降、头痛和关节痛。2周后再次入院,伴点头及
This article reports on 1 girl with central nervous system sarcoidosis and reviewed 23 CNS manifestations of childhood sarcoidosis. Children, female, 13 years old, admitted to hospital due to fever, malaise, joint pain, vomiting and weight loss for one month. Visually blurred vision for days before admission. Body temperature 38.5 ℃, pulse 80 beats / min, breathing 24 beats / min, blood pressure 120 / 80mmHg, extensor side visible obvious pigmentation. Scleral hyperemia, photophobia. Slit lamp examination showed obvious granulomatous uveitis. Cardiopulmonary no abnormalities, 4 cm below the right lateral margin of the liver, 8800 leukocytes, 30% of the rod nucleus, 34% of the neutral, 26% of the lymphatic, and 7% of the eosinophils. ESR 64mm / L hour, CPK 179 U / L, urinary protein (+), microscopic examination of leukocytes 3-5 / high times. Lung function test was significantly reduced, vital capacity of 30% of normal. Conjunctival or liver biopsy was negative. Clinical diagnosis of sarcoidosis, began with prednisone 40mg / day, discharge treatment.
