[目的]比较单肢站立膝关节屈曲45°后前位与双膝伸直站立前后位两种不同的X线摄片方法诊断膝关节骨性关节炎的敏感性.[方法]17例膝关节骨性关节炎的病人分别在相同条件下拍摄双膝伸直站立前后位和患侧单肢站立膝关节屈曲45°后前位片,在X线片上测量最小关节间隙,所得数据用SPSS 11.5软件做统计处理.[结果]17例内侧胫股间隙骨性关节炎的最小关节间隙平均值在双膝伸直站立前后位和单肢站立膝关节屈曲45°后前位上分别是(4.65±1.44)mm和(2.52±1.69)mm,差异有显著性意义(P<0.001).14例外侧胫股间隙骨性关节炎最小关节间隙在双膝伸直站立前后位和单肢站立膝关节屈曲45°后前位上分别是(6.23±1.02)mm和(4.63±1.84)mm,差异有显著性意义(p=0.004).[结论]单肢站立膝关节屈曲45°后前位摄片与双膝伸直站立前后位相比,更敏感地反映骨关节炎关节间隙变化,有望成为膝关节骨性关节炎X线检查的理想方法.“,”[Objective]Posteroanterior weight-bearing radiographs, made with the knee in 45 degrees of flexion while the patient was standing on the homolateral foot, were compared with the weight - bearing anteroposterior radiographs with both knees in full extention, in order to find a more sensitive method to detect knee osteoarthritis.[Methods]Seventeen osteoarthritis knees were radiographed in the weight - bearing anteroposterior view with both knees in full extention and in the weight -bearing posteroanterior view with the knee in 45 degrees of flexion while standing on the homolateral foot. The minimum joint space width of all the radiograph were measured. The data were analysed statistically with SPSS 11.5.[Results]The mean minimum joint space width of the medial compart in extended knee view and standing on the homolateral foot were 4. 65 mm ±1.44 mm and 2. 52 mm ± 1.69 mm, respectively (P<0.001 ) . The mean minimum joint space width of the lateral compartment were 6.23 mm±1.02 mm and 4.63 mm±1.84 mm, respectively (P=0.004) .[Conclusion]The forty - five - degree posteoanterior flexion weight - bearing radiograph of the knee while standing on the homolateral foot is a more sensitive method for the evaluation of joint space width in femorotibial osteoarthritis.
