目的 探讨颞下颌关节真性强直的手术治疗和避免术后复发。方法 采用自体肋骨、肋软骨移植对 2 7例颞下颌关节真性强直患者进行了关节重建术 ,术后随访 ,计算手术前后张口度的差值 ,检查咬合关系、下颌前伸和侧向运动功能 ,行关节X线检查。结果 2 7例随访 1～ 1 0年术前后张口度的差值均在 2 0cm以上 ,效果优良 ,重建关节无骨质吸收 ,咬合关系良好 ,下颌具有前伸和侧向运动功能 ,无复发。结论 自体肋骨、肋软骨移植的颞下颌关节重建术是治疗颞下颌关节真性强直的一种较理想方法
Objective To investigate the surgical treatment of true temporomandibular joint (TJD) ankylosis and to avoid postoperative recurrence. Methods Twenty-seven patients with true temporomandibular joint ankylosis underwent arthroscopic reconstruction using self-ribbed and costal cartilage transplantation. The difference in mouth opening before and after operation was calculated and the occlusal relationship, mandibular extension and lateral motor function were examined. X-ray examination. Results Twenty seven patients were followed up for 1 ~ 10 years before and after the difference in the mouth opening degree of more than 20cm, the effect is good, reconstruction of the joint without bone absorption, good occlusion, mandibular advancement and lateral motor function without recurrence. Conclusion Self-rib and costal cartilage transplantation of temporomandibular joint reconstruction is a more ideal method for the treatment of true rigidity of temporomandibular joint
