目的探讨乳腺良、恶性肿瘤的超声图像特点,以提高超声诊断的准确率。方法 40例乳腺肿瘤患者,其中良性肿瘤25例,恶性肿瘤15例,总结分析其二维超声及彩色多普勒(CDFI)超声图像特点。结果经超声诊断乳腺癌18例,病理证实15例,误诊3例,其中2例乳腺脓肿、1例为乳腺增生;超声诊断乳腺良性肿瘤22例,病理证实为19例,漏诊3例,1例硬癌、2例髓样癌误诊为乳腺纤维腺瘤。乳腺良性肿瘤形态规则,多具有完整包膜,内部回声均匀,CDFI示肿瘤内无或少许血流信号,血流动力指数(PI)<1.2,血流阻力指数(RI)<0.7;乳腺恶性肿瘤边界粗糙,多呈蟹足样改变,内部回声不均匀,多具有微小钙化,并伴有同侧腋窝淋巴结肿大,肿瘤内部血流多较丰富,PI>1.2,RI>0.7。结论超声检查对乳腺肿瘤具有较高的检出率,对良、恶性肿瘤的鉴别诊断具有重要意义。
Objective To investigate the characteristics of ultrasound images of benign and malignant breast tumors in order to improve the accuracy of ultrasonic diagnosis. Methods Forty cases of breast cancer, including 25 cases of benign tumors and 15 cases of malignant tumors, were analyzed retrospectively. The features of two-dimensional ultrasound and color doppler imaging were analyzed. Results Ultrasound diagnosis of breast cancer in 18 cases, pathologically confirmed in 15 cases, misdiagnosed in 3 cases, 2 cases of breast abscess, 1 case of breast hyperplasia; ultrasound diagnosis of benign breast tumor in 22 cases, 19 cases of pathological confirmed, missed diagnosis in 3 cases, 1 case Hard cancer, 2 cases of medullary carcinoma misdiagnosed as breast fibroadenoma. Breast benign tumor morphology, more with a complete envelope, internal echo uniform, CDFI showed no or little tumor blood flow signal, the hemodynamic index (PI) <1.2, blood flow resistance index (RI) <0.7; Rough boundaries, mostly crab-like changes, internal echo uneven, mostly with small calcification, accompanied by ipsilateral axillary lymph nodes, blood flow within the tumor more abundant, PI> 1.2, RI> 0.7. Conclusion Ultrasonography has a high detection rate for breast tumors and is of great significance in the differential diagnosis of benign and malignant tumors.
