目的:探讨OPN、CD44V6抗原的表达与子宫内膜样腺癌临床病理特征的关系。方法:应用免疫组化SP法对60例子宫内膜样腺癌标本进行OPN和CD44V6抗原检测。结果:OPN在增生性子宫内膜、分泌性子宫内膜及子宫内膜样腺癌中的表达率依次升高,组间差异有统计学意义,P<0.01;OPN在子宫内膜样腺癌Ⅰ~Ⅱ期和Ⅲ~Ⅳ期中的表达率分别为70.59%和92.31%,差异有统计学意义,P<0.05;在转移组中OPN的表达率为95.00%,无转移组表达率为72.50%,转移组的表达率高于无转移组,P<0.05。CD44V6在子宫内膜样腺癌中的表达高于增生性子宫内膜和分泌性子宫内膜,P<0.05;CD44V6在子宫内膜样腺癌Ⅰ~Ⅱ期和Ⅲ~Ⅳ期中的表达率分别为52.63%和81.82%,分期越高表达率越高,差异有统计学意义,P<0.05;CD44V6转移组表达率为85.00%,无转移组组表达率为52.50%,转移组的表达率高于非转移组,P<0.05;肌层浸润>1/2组表达率为85.71%,肌层浸润≤1/2组表达率为51.28%,随浸润深度的增加表达率升高,P<0.05。结论:OPN、CD44V6高表达提示子宫内膜样腺癌生物学行为不良,联合检测OPN、CD44V6可作为判断子宫内膜样腺癌恶性程度、预测转移和评估预后的生物学指标。
Objective: To investigate the relationship between the expression of OPN and CD44V6 antigen and clinicopathological features of endometrial adenocarcinoma. Methods: Immunohistochemical SP method was used to detect OPN and CD44V6 in 60 cases of endometrial adenocarcinoma. Results: The expression rates of OPN in proliferative endometrium, secretory endometrium and endometrioid adenocarcinoma were significantly higher than those in control group (P <0.01). The expression of OPN in endometrioid adenocarcinoma The expression rates of OPN in stage Ⅰ ~ Ⅱ and stage Ⅲ ~ Ⅳ were 70.59% and 92.31% respectively, the difference was statistically significant (P <0.05). The expression rate of OPN in metastasis group was 95.00%, the expression rate in non-metastasis group was 72.50% , The expression rate of metastasis group was higher than that of no metastasis group, P <0.05. The expression of CD44V6 in endometrial adenocarcinoma was higher than that in proliferative endometrium and secretory endometrium, P <0.05. The expression rates of CD44V6 in stage Ⅰ ~ Ⅱ and stage Ⅲ ~ Ⅳ of endometrial adenocarcinoma were respectively Which was 52.63% and 81.82%, respectively. The higher the staging was, the higher the expression rate was (P <0.05). The expression rate of CD44V6 in metastasis group was 85.00% and that in non-metastasis group was 52.50% In non-metastasis group, P <0.05; the expression rate of myometrial invasion> 1/2 was 85.71%, the expression rate of myometrial invasion ≤ 1/2 was 51.28%, the expression rate was increased with the depth of invasion, P <0.05 . Conclusion: The high expression of OPN and CD44V6 suggest the poor biological behavior of endometrial adenocarcinoma. Combined detection of OPN and CD44V6 can be used as a biological indicator to predict the prognosis of endometrioid adenocarcinoma.
