目的：分析各副鼻窦慢性炎症与窦口鼻道复合体病变的关系。方法：临床初诊为慢性副鼻窦炎的患者９９例行高分辨率ＣＴ扫描，对窦口鼻道复合体区的病变及各副鼻窦慢性炎症进行比较。结果：通过影像学资料分析得出副鼻窦慢性炎症表现为①漏斗型２４例，②窦口鼻道复合体型２４例，③蝶筛隐窝型７例，④息肉型４４例。结论：窦口鼻道复合体区是炎症产生的易发部位同时也是使副鼻窦诸窦腔产生慢性炎症的初发部位，随病程的增长，病变及累及的范围逐渐加重。形成，从而不可避免地阻塞窦口鼻道复合体区形成炎症，久而久之阻塞鼻窦入口特别是中鼻道，其结果导致鼻窦的慢性炎症的产生。鼻息肉是中鼻道的多发病，鼻息肉的原发部位并不在筛窦腔内，而是以筛骨在中鼻道的暴露部位为主，如钩突、上颌窦口、筛泡表面等。鼻息肉的产生能引起整个中鼻道乃至整个鼻道的闭塞，使全部副鼻窦排泄受阻，从而产生全副鼻窦慢性炎症病变。临床以用鼻内窥镜手术的方法治疗慢性副鼻窦炎，其手术的基本原理是清除副鼻窦引流通道的堵塞性病变，使该区重新恢复引流排泄功能，同时开放病变鼻窦，清除病变组织。其手术具有损伤小、操作精确、病变清除彻底等优点。本文少年组窦口鼻道复合体区呈炎性改变７例，中鼻道息肉样改变１９例。?
Objective: To analyze the relationship between chronic inflammation of paranasal sinuses and the pathology of ostiomeatal complex. Methods: Ninety-nine patients with newly diagnosed chronic sinusitis were enrolled in this study. High-resolution CT scans were performed to compare the pathological changes of sinus ostiomeatal complex and chronic inflammation of paranasal sinuses. Results: According to the imaging data analysis, chronic inflammation of accessory paranasal sinus was found in 24 cases of funnel type, 24 cases of sinus ostiomeatal complex type, 7 cases of butterfly stenosis and 4 cases of polyp type. CONCLUSION: The ostiomeatal complex area is the predilection site of inflammation and also the initial site of chronic inflammation in sinus cavities of paranasal sinuses. With the increase of the course of the disease, the range of lesion and involvement gradually aggravates. Formation, and thus inevitably block the formation of inflammation of the ostiomeatal complex, the passage of time blocking the entrance of the sinuses, especially the middle nasal passages, resulting in the production of chronic inflammation of the sinuses. Nasal polyps is the incidence of multiple nasal passages, nasal polyps, the primary site is not in the ethmoid sinus cavity, but the exposed part of the ethmoid bone in the middle of the main nasal passages, such as the uncinate process, maxillary sinus mouth, bubble surface . The production of nasal polyps can cause occlusion of the entire middle or even entire nasal passages, obstructing the excretion of all paranasal sinuses and resulting in chronic inflammation of the entire paranasal sinuses. Clinical treatment of chronic sinusitis by endoscopic sinus surgery, the basic principle of surgery is to clear the obstructive lesions of the paranasal sinus drainage channel, so that the area to resume drainage and excretion, while opening up the lesion sinus, remove the diseased tissue. The surgery has the advantages of less damage, precise operation and complete removal of lesions. This juvenile group sinus ostiomeatal complex was inflammatory changes in 7 cases, 19 cases of polypoid change in the middle. ?
