目的 寻找一种快捷、高效的应急治疗哮喘的方法。方法 将 5 0例中、重度哮喘急性发作患儿随机分为三组。A组 :超声雾化吸入 0 5 %备劳特液每次 5 0 μg/kg ,加用地塞米松 5mg ,雾化吸入 15min ;B组 :6L/min氧驱动雾化吸入 0 5 %备劳特液每次 5 0 μg/kg ,加用地塞米松 5mg ,雾化吸入 10min ;C组 :静滴氨茶碱每次 35mg/kg ,加用琥珀酸氢考每次 5 0mg/kg ,同时配合吸氧。观察各组临床症状改善情况 ,测定治疗前、后 15min最大呼气流速(PEF)。结果 ①各组临床症状改善情况 :A组改善率 2 1 4% (3/14) ;B组改善率 85 0 % (17/2 0 ) ;C组改善率81 3% (13/16 )。②各组治疗前后ΔPEF(L/min)变化 :A组 11 714± 7 5 49,B组 41 10 0± 2 6 18,C组 40 875±19 418;③各组PEF变异率 >15 %情况 :A组阳性率 7 1% (1/14) ,B组阳性率 6 5 0 % (13/2 0 ) ,C组阳性率 6 8 8%(11/16 )。B、C两组临床症状改善和ΔPEF改变优于A组 (P <0 0 1)。结论 6L/min氧驱动雾化吸入治疗中、重度哮喘急性发作是一个快速、高效的方法
Objective To find a quick and efficient emergency treatment of asthma. Methods Fifty cases of acute exacerbation of moderate and severe asthma were randomly divided into three groups. Group A: ultrasonic atomization inhalation 0 5% spare Lute liquid each 50 μg / kg, plus dexamethasone 5mg, inhalation 15min; Group B: 6L / min oxygen driven inhalation 0 5% 50 μg / kg each time plus dexamethasone 5 mg and inhalation for 10 min. Group C: intravenous aminophylline 35 mg / kg each time, plus 50 mg / kg hydrogen succinate and 50 mg / oxygen. The improvement of clinical symptoms was observed in each group, and the maximum expiratory flow rate (PEF) was measured before and 15 minutes after treatment. Results ① The improvement of clinical symptom in each group: the improvement rate of group A was 21.4% (3/14); the improvement rate of group B was 85 0% (17/2 0); the improvement rate of group C 81 3% (13/16). ② Changes of ΔPEF (L / min) before and after treatment in each group: 11 714 ± 7 5 49 in group A, 41 10 ± 2 6 18 in group B and 40 875 ± 19 418 in group C; ③ The PVE mutation rate in each group was> 15% The positive rate of group A was 71% (1/14), that of group B was 65.0% (13/2), and that of group C was 68.8% (11/16). The improvement of clinical symptoms and changes of ΔPEF were better in group B and C than in group A (P <0.01). Conclusion 6L / min oxygen-driven inhalation therapy for moderate and severe acute exacerbation of asthma is a rapid and efficient method
