目的发挥病区药房主管药师合理用药把关的一线屏障作用,建立病区药房处方点评制度,直接干预不合理用药。方法在《医院处方点评管理规范(试行)》的具体指导下,建立一般点评和专项点评制度,制定改进措施,持续改进。结果通过一般点评和专项点评制度的实践,处方质量明显改进,药师点评处方的能力相应提高,对不合理用药的干预有理有据,得到了临床的认可。结论病区药房实行一般点评和专项点评制度,发挥一线屏障作用,对不合理用药进行干预,将不合理用药等不安全因素消灭在萌芽状态,提高了临床用药的安全性、合理性、有效性。
Aim To play a first-line barrier effect of pharmacy competent pharmacists in ward pharmacies to establish a prescription review system for pharmacies in the wards and directly interfere with irrational drug use. Methods Under the specific guidance of “Management Regulations of Hospital Prescription Reviews (for Trial Implementation)”, a general comment system and a special comment system were established, improvement measures were formulated, and continuous improvement was made. Results Through the practice of general comments and special comment system, the quality of prescriptions was significantly improved. The ability of pharmacists to comment on prescriptions was correspondingly improved. The intervention on irrational drug use was well-founded and was clinically approved. Conclusion The general review and special comment system in the pharmacy of ward is implemented. It plays a first-line barrier function, interferes with unreasonable medication, eliminates the unscientific factors such as irrational medication and other unsafe factors and improves the safety, rationality and effectiveness of clinical medication .
