目的：描述小儿盘状半月板及损伤的ＭＲ特点及异常ＭＲ信号之光镜所见，提出小儿盘状半月板诊断标准。材料与方法：回顾性分析２４例经手术证实的小儿盘状半月板伴损伤的ＭＲ所见，与１６例正常志愿者对照，并将半月板内异常信号与光镜下对照。结果：冠状位测量外侧盘状半月板体部中央横径平均２８５ｍｍ（对照组１２８３ｍｍ）与胫骨平台横径之比为３５７８％（对照组２０％），同层半月板中点高径为４７８ｍｍ，对照组３７６ｍｍ）。检验结果均有显著性差异。ＭＲ上ＲｅｉｃｈｅｒⅠ度异常信号光镜下为纤维软骨的粘液样变性。ＩＶ度表现为撕裂的半月板边缘除发生退变外还可见大量滑膜细胞及少许毛细血管。结论：１半月板体部中央横径超过２０ｍｍ，与胫骨平台之比大于３０％，附以形态学特征，即可诊断盘状半月板。２ＭＲＩ是诊断小儿盘状半月板及损伤的理想检查方法。
OBJECTIVE: To describe pediatric traumatic meniscus and the MR characteristics of the trauma and abnormal MR signals seen by the light microscope, put forward pediatric discoid meniscus diagnostic criteria. Materials and Methods: A retrospective analysis of 24 cases of pediatric disc-shaped meniscus confirmed by surgery with MR findings, and 16 normal volunteers, and the abnormal signal within the meniscus and light microscopy. Results: Coronal measurement of the average lateral diameter of the central discotic meniscus 28.5mm (control group 12.83mm) and the tibial plateau diameter ratio of 35.78% (control group 20%), the same layer in the meniscus Point height of 4 78mm, control group 3 76mm). The test results were significant differences. Myocardial degeneration of fibrocartilage under the Reicher Ⅰ degree abnormal signal light microscope in MR. IV degree showed the edge of the torn meniscus in addition to degeneration but also a large number of synovial cells and a small number of capillaries. Conclusions: 1. The central transverse diameter of the body of the meniscus exceeds 20 mm and the ratio to the tibial plateau is more than 30%. With the morphological features, the discoid meniscus can be diagnosed. 2 MRI diagnosis of pediatric discoid meniscus and the ideal method of inspection.
