目的:探讨臂踝脉搏波传导速度(baPWV)与心血管危险因素及国人缺血性心血管疾病(ICVD)危险评分的相关性。方法:2010年3月~7月天津市人民医院体检人群232例进行常规体检,应用全自动动脉硬化测定仪VBP-9测定双肢的baPWV,按代谢异常组分进行分组,分别比较各组临床基本资料、baPWV及ICVD风险评分。结果:随着代谢异常组分的增加,baPWV逐渐增加,代谢异常各组baPWV均高于对照组,各组间差异均有统计学意义(P<0.05)。不同组之间,体质量指数、收缩压、舒张压、空腹血糖、三酰甘油、10年风险评估随着代谢异常数目的增高而呈上升趋势,高密度脂蛋白胆固醇(HDL-C)随着代谢异常数目的增加而降低,各组间差异具有统计学意义(P<0.05),糖尿病史、高血压病史、吸烟史各组间差异有统计学意义(P<0.05)。Logistic回归分析:baPWV影响因素中年龄OR=1.062 95%C I(1.009-1.118)、收缩压OR=1.085 95%C I(1.032-1.139)、糖尿病史OR=5.650 95%C I(1.735-18.395);国人ICVD危险评分与baPWV存在回归关系(r=0.531,P<0.01)。结论:年龄、收缩压、糖尿病史是早期动脉粥样硬化的影响因素,各种主要心血管危险因素与baPWV测定值相关,ICVD评分与baPWV相关。
Objective: To investigate the relationship between brachial-ankle pulse wave velocity (baPWV) and cardiovascular risk factors and ischemic cardiovascular disease (ICVD) risk score. Methods: From March to July 2010, 232 cases of physical examination in Tianjin People’s Hospital were examined by routine physical examination. The baPWV of both limbs was measured by automatic arteriosclerosis analyzer VBP-9. According to the abnormal metabolic components, Basic data, baPWV and ICVD risk score. Results: With the increase of metabolic abnormalities, the baPWV increased gradually, while the abnormal baPWV in each group was higher than that in the control group. There was significant difference between the two groups (P <0.05). The body mass index, systolic blood pressure, diastolic blood pressure, fasting blood glucose, triglyceride, 10-year risk assessment showed an upward trend with the increase of the number of metabolic abnormalities among different groups. With the increase of high density lipoprotein cholesterol (HDL-C) (P <0.05). There was a significant difference between the two groups in the history of diabetes mellitus, hypertension and smoking history (P <0.05). Logistic regression analysis showed that among the influencing factors of baPWV, the OR = 1.062 95% CI (1.009-1.118), the systolic blood pressure OR = 1.085 95% CI (1.032-1.139), the history of diabetes OR = 5.650 95% CI (1.735-18.395) There was a regression relationship between ICVD risk score and baPWV (r = 0.531, P <0.01). CONCLUSIONS: Age, systolic blood pressure and diabetes mellitus are the influencing factors of early atherosclerosis. Various major cardiovascular risk factors are correlated with the measured values of baPWV, and ICVD scores are correlated with baPWV.
