目的 :研究 2型糖尿病及糖耐量减低患者血清瘦素水平与肥胖、血糖、胰岛素水平、胰岛素抵抗指数及血脂水平的关系。方法 :将 3 0例 2型糖尿病患者和 90例糖耐量减低患者按体重指数分为 3组 :正常体重组、超重组、肥胖组。分别测定瘦素、空腹胰岛素、餐后2 h胰岛素、体重指数、腰围、臀围、腰臀比值、甘油三酯、总胆固醇和高密度脂蛋白。结果 :LEP与 BMI、WAIST、HIP、FINS、PINS、IRI、TG呈正相关 ,而与 FBG呈负相关 ;LEP与PBS、TCH、HDL-c无关。肥胖组的 LEP、FINS、PINS、IRI水平明显高于超重组和正常体重组。结论 :2型糖尿病和糖耐量减低患者的瘦素水平受肥胖、高胰岛素水平、胰岛素抵抗和血脂等多因素的影响 ,提示瘦素可能在 2型糖尿病及糖耐量减低患者的发病中起一定的作用。
Objective: To study the relationship between serum leptin level and obesity, blood glucose, insulin level, insulin resistance index and blood lipid level in patients with type 2 diabetes and impaired glucose tolerance. Methods: 30 patients with type 2 diabetes mellitus and 90 patients with impaired glucose tolerance were divided into 3 groups according to body mass index: normal weight group, overweight group and obesity group. Leptin, fasting insulin, 2 h postprandial insulin, body mass index, waist circumference, hip circumference, waist-hip ratio, triglyceride, total cholesterol and high density lipoprotein were measured. Results: LEP was positively correlated with BMI, WAIST, HIP, FINS, PINS, IRI and TG, but negatively correlated with FBG. LEP was not related with PBS, TCH and HDL-c. The levels of LEP, FINS, PINS and IRI in obesity group were significantly higher than those in overweight group and normal weight group. CONCLUSION: Leptin levels in type 2 diabetes and impaired glucose tolerance patients are affected by many factors, including obesity, high insulin level, insulin resistance and blood lipids, suggesting that leptin may play a role in the pathogenesis of type 2 diabetes and impaired glucose tolerance effect.
