目的通过观察窒息新生儿血清磷酸肌酸激酶同功酶(CK-MB)值的变化,探讨窒息新生儿缺氧程度与心肌损害程度之间的关系。方法将2010年1月至2013年2月田阳县人民医院新生儿病房住院的40例窒息新生儿(观察组)的CK-MB值,与40例无窒息的新生儿(对照组)的CK-MB值进行对比分析。结果观察组的平均血清CK-MB值明显高于对照组;观察组中重度窒息患儿平均血清CK-MB值明显高于轻度窒息患儿;窒息新生儿日龄越小,其血清CKMB值越高,随着治疗而逐渐下降;观察组早产窒息新生儿血清CK-MB值明显高于对照组的早产儿,且明显高于观察组全组及重度窒息新生儿的平均血清CK-MB值。结论窒息新生儿的缺氧程度与心肌损害程度之间的关系呈正相关,此关系在早产儿中的表现更加明显;血清CK-MB值的变化可作为窒息新生儿心肌损害早期诊断敏感而特异的指标,具有判断病情轻重、指导临床治疗及判断疾病转归的重要意义。
Objective To investigate the relationship between the level of myocardial hypoxia and the degree of hypoxia in neonates with asphyxia by observing the change of serum creatine phosphokinase (CK-MB) in asphyxial neonates. Methods The CK-MB values of 40 asphyxiated newborns (observation group) hospitalized in neonatal wards of Tianyang People’s Hospital from January 2010 to February 2013 were compared with CK-MB of 40 asphyxia neonates (control group) MB value for comparative analysis. Results The mean serum CK-MB value of the observation group was significantly higher than that of the control group. The mean serum CK-MB value of the children with severe asphyxia in the observation group was significantly higher than that of the mild asphyxia group. The lower the neonatal asphyxia was, the lower the serum CKMB value The value of serum CK-MB in observation group was significantly higher than that in control group, and was significantly higher than that in the observation group and the group with severe asphyxia . Conclusions There is a positive correlation between the degree of hypoxia and the degree of myocardial damage in neonates with asphyxia, and this relationship is more obvious in preterm infants. The change of serum CK-MB value may be sensitive and specific for early diagnosis of myocardial damage in asphyxia neonates Indicators, with judgment severity, guiding clinical treatment and prognosis of the importance of the disease.
