目的:探讨Willis覆膜支架治疗颈内动脉虹吸部动脉瘤的临床疗效。方法:回顾性分析2015年10月至2018年12月同济大学附属同济医院介入科采用Willis覆膜支架治疗的62例颈内动脉虹吸部动脉瘤患者(共62个动脉瘤)的临床资料。62个动脉瘤中,33个破裂,另29个未破裂;45个采用单覆膜支架置入术,2个采用双覆膜支架置入术,12个采用单覆膜支架联合弹簧圈栓塞术,另3个采用双覆膜支架联合弹簧圈栓塞术。出院后对所有患者行临床随访,术后3个月复查头颅数字减影血管造影(DSA),并采用改良Rankin量表评分(mRS)评估患者神经功能状态。结果:62例患者的62个动脉瘤均一次成功置入覆膜支架。62个动脉瘤术后即刻造影显示,53个(85%)动脉瘤完全闭塞,另9个(15%)出现内漏,其中7个表现为轻度动脉瘤瘤腔内显影并滞留。62例患者中,术后3例(4.8%)出现手术操作相关并发症,其中1例出现支架移位,1例出现支架内急性血栓形成,另1例因术中血管穿孔致颅内出血死亡;6例(9.7%)发生眼动脉闭塞,但均未出现视力下降或视野缺损症状。61例患者的随访时间为(9.0±4.7)个月(3~24个月)。随访期间,61例患者均未发生脑缺血或出血性事件,且均未发现动脉瘤复发。术后3个月,49例患者进行DSA随访,其中46个(94%)动脉瘤完全闭塞,另3个(6%)出现内漏。至末次随访,61例患者中,60例mRS为0~2分,1例为4分。结论:初步判断Willis覆膜支架治疗破裂或未破裂颈内动脉虹吸部动脉瘤的完全闭塞率较高,内漏发生率较低,且患者术后短期内神经功能恢复较好。“,”Objective:To investigate the efficacy of Willis covered stent in the treatment of internal carotid artery siphon aneurysms.Methods:The data of 62 cases with 62 internal carotid artery siphon aneurysms treated with Willis covered stent at Department of Interventional Radiology, Tongji Hospital Affiliated to Tongji University from October 2015 to December 2018 were retrospectively analyzed. Among them, 33 were ruptured and 29 were unruptured aneurysms. Single covered stent was employed in 45 aneurysms, double covered stents in 2, single covered stents plus coil embolization in 12, and double covered stents plus coil embolization in 3. All patients were followed up clinically after discharge. Cerebral digital subtraction angiography (DSA) was performed after 3 months, and the modified Rankin scale (mRS) was used to assess the neurological status of the patients.Results:All 62 patients were successfully treated with covered stents. Immediate postoperative angiography revealed complete occlusion of aneurysm in 53 (85%) cases and endoleak in 9 (15%) cases, 7 of which presented mind retention of contrast agent in the aneurysm cavity. Out of 62 cases, procedure-related complications occurred in 3 (4.8%) and included stent displacement in 1, acute in-stent thrombosis in 1 and intracranial hemorrhage due to intraoperative vessel rupture which resulted in death in 1. Ophthalmic artery occlusion occurred in 6 (9.7%) patients, while no defect of visual acuity or visual field was observed. The follow-up duration of 61 patients was 9.0±4.7 months and ranged from 3 to 24 months, during which there was no ischemic, hemorrhagic event or aneurysm recurrence. The DSA follow-up was performed in 49 patients after 3 months, which demonstrated that complete aneurysm occlusion was reported in 46 (94%) patients and endoleak occurred in 3(6%). The mRS score was 0-2 in 60 patients and 4 in 1.Conclusion:This preliminary study suggests that Willis covered stent used to treat internal carotid siphonal aneurysms is associated with a relatively high rate of complete occlusion of aneurysm, a relatively low rate of endoleak, and good improvement of neurological function as well.
