目的:探讨E-钙粘蛋白(E-cadherin,E-cad)的表达与胃癌的浸润、转移及预后的关系。方法:应用SP免疫组化方法检测76例胃癌E-cad的表达。结果:正常胃粘膜上皮中E-cad全部表达。Ⅰ期E cad的异常表达率为19.6%,而Ⅱ、Ⅲ、Ⅳ期的异常表达率分别是57.2%、62.1%和69.3%,Ⅰ期和Ⅱ、Ⅲ、Ⅳ期间有非常显著性差异(P<0.01)。伴有淋巴结转移者51例中异常表达为74.3%,而无淋巴结转移者40.2%,两者间有非常显著性差异(P<0 .05)。在进展期胃癌中按肿瘤生长方式分组,结节型E cad异常表达率41.2%,而溃疡浸润型异常表达率67.3%,两者间有显著性差异(P<0.05);高中分化胃癌E cad异常表达率32 .0%,而低分化胃癌E cad异常表达率69.6%,两者间有非常显著性差异(P<0 01)。癌细胞浸润深度达粘膜、粘膜下层和肌层者E cad异常表达率为30.1%、38.2%,而浸润至浆膜及浆膜外者异常率为67.3%,两者间有非常显著性差异(P<0.01)。胃癌组织中E-cad的表达与癌组织分化、浸润和转移密切相关。结论:E-cad与肿瘤的发生、浸润、转移有关,可作为一种新的肿瘤标记物,对判断预后有一定价值。
Objective: To investigate the relationship between E-cadherin (E-cad) expression and infiltration, metastasis and prognosis of gastric cancer. Methods: The expression of E-cadherin in 76 cases of gastric cancer was detected by SP immunohistochemistry. Results: E-cad in normal gastric mucosa epithelium was all expressed. The abnormal expression rate of stage Ⅰ E cad was 19.6%, while the abnormal rates of stage Ⅱ, Ⅲ and Ⅳ were 57.2%, 62.1% and 69.3% respectively. There was a significant difference between stage Ⅰ and Ⅱ, Ⅲ and Ⅳ (P <0.01). Of the 51 cases with lymph node metastasis, the abnormal expression was 74.3%, but no lymph node metastasis was 40.2%, there was a significant difference between the two (P <0.05). In advanced gastric cancer grouping according to tumor growth, the abnormal expression rate of nodular E cad was 41.2%, but the abnormal expression rate of invasive ulceration was 67.3%, there was a significant difference between the two (P <0.05) The abnormal expression rate was 32.0%, while the abnormal expression rate of E cad in poorly differentiated gastric cancer was 69.6%. There was a significant difference between the two (P <0.01). The abnormal expression rate of E cad in cancerous cells reaching the mucosa, submucosa and myometrium was 30.1% and 38.2%, while the abnormal rate of infiltration into the serosa and serosa was 67.3%, with significant difference between the two P <0.01). The expression of E-cad in gastric cancer is closely related to the differentiation, invasion and metastasis of cancer tissue. Conclusion: E-cad is associated with the occurrence, invasion and metastasis of tumor, which can be used as a new tumor marker and has certain value in judging the prognosis.
