目的分析研讨左卡尼汀注射液治疗老年冠心病心力衰竭(简称“心衰”)的临床疗效。方法选取68例2014年7月—2015年12月衡阳市中心医院收治的老年冠心病心衰患者,随机分为对照组和观察组,各34例,对照组接受常规治疗(硝酸脂类药物、利尿剂、血管紧张素转换酶抑制剂及β受体阻滞剂等),观察组在对照组治疗基础上加用左卡尼汀注射液。将两组患者的临床疗效、左室舒张末期容积(LVEDD)、左室收缩末期容积(LVESD)、左室射血分数(EF)、心输血量(CO)等心功能指标及空腹血糖(FPG)、餐后血糖(PPG)、糖化血红蛋白(Hb A1c)水平进行对比分析。结果经治疗后,观察组(94.12%)总有效率明显高于对照组(79.14%),差异有统计学意义(P<0.05);观察组患者的LVEDD、LVESD、EF、CO等心功能指标改善情况明显优于对照组,差异有统计学意义(P<0.05);观察组FPG、PPG、Hb A1c水平均明显优于对照组,差异有统计学意义(P<0.05)。结论左卡尼汀注射液治疗老年冠心病心力衰竭,其疗效确切,可明显改善患者的心功能,值得临床应用并推广。
Objective To investigate the clinical efficacy of L-carnitine injection in the treatment of heart failure of elderly patients with coronary heart disease (referred to as “heart failure”). Methods Sixty-eight elderly patients with CHD who were admitted to Hengyang Central Hospital from July 2014 to December 2015 were randomly divided into control group and observation group, 34 cases in each group. The control group received routine treatment (nitric-lipid drugs, Diuretics, angiotensin converting enzyme inhibitors and β-blockers, etc.), the observation group in the control group based on the treatment with levocarnitine injection. The clinical efficacy, left ventricular end-diastolic volume (LVEDD), left ventricular end systolic volume (LVESD), left ventricular ejection fraction (EF), cardiac output (CO) and other cardiac function and fasting blood glucose ), Postprandial blood glucose (PPG) and glycosylated hemoglobin (Hb A1c) levels were compared. Results After treatment, the total effective rate of the observation group (94.12%) was significantly higher than that of the control group (79.14%), the difference was statistically significant (P <0.05); LVEDD, LVESD, EF, The improvement was significantly better than that of the control group (P <0.05). The levels of FPG, PPG and Hb A1c in the observation group were significantly better than those in the control group (P <0.05). Conclusion L-carnitine injection treatment of heart failure in elderly patients with coronary heart disease, the exact effect, can significantly improve the patient’s cardiac function, it is worthy of clinical application and promotion.
