目的 探讨大肠癌外周血循环癌细胞检测的临床意义。 方法 以CK-20 mRNA为靶分子,RT-PCR技术检测3种癌细胞株(结肠腺癌细胞株SW-480、胃癌细胞株SGC-7901、食管癌细胞株TE-11)和67例大肠癌患者外周血细胞CK-20 mRNA的表达。 结果 三种癌细胞株中,只有结肠癌细胞表达CK-20 mRNA。RT-PCR技术能检出10~7个单核细胞中的一个肿瘤细胞;67例患者中,35例外周血CK-20 mRNA阳性,阳性率与大肠癌Dukes′分期明显相关:Dukes’A期25%,B期38.46%,C期55.17%,D期70.59%。53例手术治疗患者中,术前25例CK-20 mRNA阳性,术后30例阳性。25例术后早期短程化疗患者,化疗前阳性6例,化疗后仅2例阳性。 结论 大肠癌循环癌细胞(CK-20 mRNA)的检测可能成为判断大肠癌恶性程度、转移和复发以及疗效监测的客观指标。
Objective To explore the clinical significance of detection of circulating cancer cells in peripheral blood of colorectal cancer. METHODS: CK-20 mRNA was used as target and RT-PCR was used to detect three cancer cell lines (colonic adenocarcinoma cell line SW-480, gastric cancer cell line SGC-7901 and esophageal cancer cell line TE-11) and 67 cases of colorectal cancer. The expression of CK-20 mRNA in peripheral blood of patients. Results Of the three cancer cell lines, only colon cancer cells expressed CK-20 mRNA. One of the 10 to 7 mononuclear cells was detected by RT-PCR. Among the 67 patients, 35 were positive for CK-20 mRNA in the peripheral blood. The positive rate was significantly associated with the Dukes’ stage of colorectal cancer: Dukes’A stage. 25%, 38.46% in period B, 55.17% in period C, and 70.59% in period D. Of the 53 surgical patients, 25 cases were positive for CK-20 mRNA before surgery and 30 cases were positive after surgery. Of the 25 patients who underwent early postoperative short-course chemotherapy, 6 were positive before chemotherapy and only 2 were positive after chemotherapy. Conclusion The detection of circulating cancer cells (CK-20 mRNA) in colorectal cancer may be an objective indicator for judging the degree of malignancy, metastasis and recurrence of the colorectal cancer and monitoring the therapeutic effect.
