目的分析高效价冷凝集素对血型鉴定的干扰及处理方法。方法用37℃生理盐水反复洗涤患者红细胞,使自身冷凝集素消失后进行正定型;在4℃条件下进行冷凝集素吸收实验,用吸收后的血浆进行反定型。结果对患者ABO血型鉴定中自身存在的高效价冷凝集素对于同时存在的同种异性抗体有遮盖作用,让血型鉴定存在较大困难甚至引发血型误判。结论当高效价冷凝集素引起患者血型鉴定困难时,应采取正确的处理方式观察患者病情,排除冷凝集素对血型鉴定的影响。
Objective To analyze the interference of high potency cold agglutinin on blood type identification and its treatment. Methods The erythrocytes were washed repeatedly with normal saline at 37 ° C to make positive self-condensation after disappearance of self-condensing lectin. The cold agglutinin absorption test was carried out at 4 ° C, and the anti-cold plasma was taken back. Results The high titer of cold agglutinin existed in the ABO blood group identification of patients has the effect of covering the same alloantibodies simultaneously, which makes the blood type identification difficult and even leads to blood type miscarriage. Conclusion When high titer of cold agglutinin causes the identification of patients with blood group difficulties, the correct treatment should be taken to observe the patient’s condition and eliminate the influence of cold agglutinin on blood group identification.
