目的探讨高血压病患者不同糖代谢下血清铁蛋白(SF)水平与胰岛β细胞功能的关系。方法纳入2009年1月至2011年6月内分泌门诊就诊的高血压病患者112例,测量体质指数(BMI)、腰围(WC),测定血清指标:空腹血糖(FPG)、胆固醇(CHO)、甘油三酯(TG)、高低密度脂蛋白胆固醇(HDL-C、LDL-C)、尿酸(UA)、空腹胰岛素(FTI)及SF,据75g口服葡萄糖耐量实验分为糖代谢正常(NGT)组、糖调节受损(IGR)组和新发糖尿病组。结果(1)新诊断2型糖尿病(糖尿病组)36例(32.1%),IGR 46例(41.1%),NGT 30例(26.8%)。(2)3组间BMI、WC、CHO、LDL-C、HDL-C比较差异无统计学意义(P均>0.05);糖尿病组的FPG、FTI、TG、SF、HOMA-IR明显高于IGR组和NGT组,UA高于NGT组而HOMA-β低于NGT组,IGR组的FPG、TG、SF、HOMA-IR明显高于NGT组,HOMA-β低于NGT组,差异均有统计学意义(P均<0.05)。(3)校正年龄和性别后的偏相关分析显示,SF与FPG(r=0.466,P<0.01)、FTI(r=0.413,P<0.01)、HOMA-IR(r=0.443,P<0.01)呈正相关。结论高血压病患者出现糖代谢异常时,SF水平明显增高,与胰岛素抵抗相关。
Objective To investigate the relationship between serum ferritin (SF) and pancreatic β-cell function in patients with essential hypertension under different glucose metabolism. Methods A total of 112 hypertensive patients admitted to the endocrine clinic between January 2009 and June 2011 were enrolled in this study. Body mass index (BMI) and waist circumference (WC) were measured. Serum parameters were measured: fasting blood glucose (FPG), cholesterol TG, HDL-C, UA, FTI and SF were divided into three groups according to 75g oral glucose tolerance test: normal glucose metabolism (NGT) group , Impaired glucose regulation (IGR) and new-onset diabetes mellitus. Results (1) 36 cases (32.1%) newly diagnosed type 2 diabetes mellitus (diabetic group), 46 cases (41.1%) of IGR and 30 cases (26.8%) of NGT. (2) There was no significant difference in BMI, WC, CHO, LDL-C and HDL-C between the three groups (all P> 0.05); FPG, FTI, TG, SF and HOMA-IR in diabetic group were significantly higher than those in IGR Group and NGT group, UA was higher than NGT group and HOMA-β was lower than NGT group, FPG, TG, SF, HOMA-IR of IGR group was significantly higher than NGT group, HOMA-β was lower than NGT group, the difference was statistically Significance (P <0.05). (3) Partial correlation analysis after age and gender correction showed that there was significant difference between SF and FPG (r = 0.466, P <0.01), FTI (r = 0.413, Was positively correlated. Conclusions When abnormal glucose metabolism occurs in hypertensive patients, the level of SF is significantly increased, which is correlated with insulin resistance.
