目的探讨核磁共振成像(MRI)在咬合板治疗颞下颌关节盘可复性前移位中的应用价值。方法颞下颌关节盘可复性前移位的患者8例(14个关节),按照咬合板制作时弹响消失的位置分为稳定咬合板治疗组(A组)和再定位咬合板治疗组(B组);患者坚持夜间戴用咬合板6个月,每月复诊1次;观察临床症状的变化和核磁共振图像,测量闭口位盘分界角。结果治疗前A组盘分界角明显小于B组[(40.46±2.95)度vs.(48.75±2.76)度](P<0.05)。与治疗前比较,治疗后两组盘分界角均明显减小为(13.27±5.83)度和(9.34±3.01)度(P<0.05)。结论 MRI技术在咬合板治疗颞下颌关节盘可复性前移位的效果评价及咬合板类型的选择中具有一定的应用价值。
Objective To investigate the value of magnetic resonance imaging (MRI) in the occlusal plate treatment of temporomandibular joint disc refolding before the application value. Methods Eight patients (14 joints) who had transposition of temporomandibular joint disc were divided into stable bite plate group (A group) and repositioned bite plate group Group B). Patients were insisted on wearing occlusal plates for 6 months at night and once a month for follow-up. Changes in clinical symptoms and magnetic resonance imaging were observed. Results Before treatment, the angle of demarcation in group A was significantly smaller than that in group B [(40.46 ± 2.95) vs. (48.75 ± 2.76) degrees] (P <0.05). Compared with those before treatment, the angle of demarcation between the two groups was significantly reduced to (13.27 ± 5.83) degrees and (9.34 ± 3.01) degrees (P <0.05), respectively. Conclusion The MRI technique has certain value in the evaluation of the effect of occlusal plate in the treatment of temporomandibular joint disc refolding and the selection of occlusal plate type.
