目的探讨正常妊娠妇女甲状腺功能指标的正常范围。方法选择2013年1月—2014年12月进行常规孕检的健康妊娠妇女623例作为研究对象,按照妊娠时间分为妊娠早期组264例、妊娠中期组227例、妊娠晚期组132例,并选择同期进行健康体检的健康非妊娠妇女作为对照组100例。观察各组游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)的变化情况和特异性参考范围。结果 FT3在妊娠早、中、晚期的特异性参考区间分别为3.34~5.73 pmol/L、3.12~5.49 pmol/L、2.93~5.42 pmol/L。FT4在妊娠早、中、晚期分别为12.61~19.03 pmol/L、8.52~17.44、8.48~17.20 pmol/L。TSH在妊娠早、中、晚期分别为0.30~4.10ml U/L、0.57~4.77 ml U/L、0.78~4.63 ml U/L。FT3的中、晚期组以及FT4、TSH的早、中、晚期组与对照组均具有统计学差异(P<0.05)。各项指标的妊娠中、晚期组与各自早期组相比较均具有统计学差异(P<0.05)。FT4的妊娠晚期组与妊娠中期组比较具有统计学差异(P<0.05)。结论妊娠妇女体内甲状腺水平与非妊娠妇女具有较大改变,制定当地的妊娠妇女甲状腺功能指标有利于妊娠妇女甲状腺疾病的诊治。
Objective To investigate the normal range of thyroid function in normal pregnant women. Methods A total of 623 healthy pregnant women who underwent routine pregnancy test from January 2013 to December 2014 were selected and divided into 264 cases of early pregnancy group, 227 cases of middle pregnancy group and 132 cases of late pregnancy group according to the time of pregnancy. The same period healthy physical examination of non-pregnant women as a control group of 100 cases. The changes of free triiodothyronine (FT3), free thyroxine (FT4), thyrotropin (TSH) and the specific reference range were observed. Results The specific reference intervals of FT3 in early, middle and late pregnancy were 3.34-5.70 pmol / L, 3.12-5.49 pmol / L and 2.93-5.42 pmol / L, respectively. FT4 in the early, middle and late pregnancy were 12.61 ~ 19.03 pmol / L, 8.52 ~ 17.44,8.48 ~ 17.20 pmol / L. TSH in early pregnancy, middle and late were 0.30 ~ 4.10ml U / L, 0.57 ~ 4.77 ml U / L, 0.78 ~ 4.63 ml U / L. FT3 middle and late groups and FT4, TSH early, middle and late groups and the control group were statistically significant (P <0.05). The indicators of pregnancy, the late group compared with their respective early groups were statistically significant (P <0.05). There was a significant difference between FT4 group and the third trimester group (P <0.05). Conclusions The level of thyroid in pregnant women has a great change with that of non-pregnant women. The development of local thyroid function indexes in pregnant women is helpful for the diagnosis and treatment of thyroid diseases in pregnant women.
