目的:探讨血浆脑钠肽(BNP)水平预测舒张早期二尖瓣血流与瓣环运动峰值速度的比值(E/Em)的价值。方法:常规超声测量左室舒张末内径、E/A比值、射血分数等。利用QTVI技术测量Em,计算6位点平均速度Em、E/Em。根据E/Em将97例受试者分为E/Em<8(17例)、E/Em 8~15(43例)、E/Em>15(37例)3组。应用ELISA法检测其血浆BNP浓度,评价血浆BNP浓度的受试者工作特征曲线(ROC曲线)预测E/Em的价值。结果:E/Em<8组、E/Em 8~15组、E/Em>15组的BNP浓度分别为(102.53±100.19)、(155.51±171.77)、(306.11±238.46)ng/L;E/Em>15组与E/Em<8组、E/Em 8~15组比较,均P<0.01;E/Em<8组与E/Em 8~15组比较,差异无统计学意义。BNP浓度与E/Em呈相关性,BNP预测E/Em>15的ROC曲线下面积为0.851(P<0.01)。结论:BNP浓度与E/Em相关密切,BNP是预测E/Em>15的较好指标。
Objective: To investigate the value of plasma brain natriuretic peptide (BNP) level in predicting the ratio of early diastolic mitral flow and annulus velocity (E / Em). Methods: Left ventricular end-diastolic diameter, E / A ratio and ejection fraction were measured by routine ultrasound. The QTVI technique was used to measure Em and the 6-site average velocity Em, E / Em was calculated. According to E / Em, 97 subjects were divided into three groups: E / Em <8 (17 cases), E / Em 8-15 (43 cases) and E / Em> 15 (37 cases). The plasma concentration of BNP was measured by ELISA, and the receiver operating characteristic curve (ROC curve) of plasma BNP concentration was used to predict the value of E / Em. Results: The BNP concentrations of E / Em <8, E / Em 8-15 and E / Em> 15 groups were (102.53 ± 100.19), (155.51 ± 171.77), (306.11 ± 238.46) ng / L, / Em> 15 and E / Em <8, E / Em 8-15, all P <0.01; there was no significant difference between E / Em <8 and E / Em 8-15 groups. The BNP concentration was correlated with E / Em. The area under the ROC curve of BNP predicted E / Em> 15 was 0.851 (P <0.01). Conclusion: BNP concentration is closely related to E / Em, and BNP is a good predictor of E / Em> 15.
