目的:比较不同n 垫式活动矫治器治疗完全性唇腭裂的效果。n 方法:抽取2017年6月至2020年6月郑州大学附属郑州中心医院收治的62例单侧完全性唇腭裂患儿作为研究对象,将牙弓主要缩窄于后部的患儿纳入螺旋组,采用螺旋扩弓器n 垫式活动矫治器;将牙弓前后部皆缩窄,且前部缩窄更明显患儿纳入扇形组,采用扇形扩弓器n 垫式活动矫治器。每组31例,均行扩弓治疗。比较两组治疗前后前牙覆盖、磨牙关系、下颌长、梨状孔最大横径、第一前磨牙区鼻腔下部最大横径、第一磨牙区鼻腔下部最大横径上齿槽座角(SNA)、下颌矢向位角(SNB)、SNA角与SNB角之差(ANB)、鼻气道体积、咽气道体积及咽气道最小横截面积。n 结果:扇形组治疗后前牙覆盖低于治疗前及对照组治疗后,差异有统计学意义(n P<0.05);两组治疗后磨牙关系低于治疗前,下颌长高于治疗前,差异有统计学意义(n P<0.05)。两组治疗后梨状孔最大横径、第一前磨牙区鼻腔下部最大横径、第一磨牙区鼻腔下部最大横径高于治疗前,差异有统计学意义(n P<0.05)。两组治疗后SNA、ANB高于治疗前,SNB低于治疗前,差异有统计学意义(n P<0.05)。两组治疗后鼻气道体积、咽气道体积、口咽气道最小横截面积高于治疗前,差异有统计学意义(n P<0.05)。n 结论:螺旋及扇形扩弓器n 垫式活动矫治器应用于完全性唇腭裂患儿,对各自适应证均能较好地扩弓,提高正畸治疗效果。n “,”Objective:To compare the effect of different occlusal pads on complete cleft lip and palate.Methods:A total of 62 children with unilateral complete cleft lip and palate treated in Zhengzhou Central Hospital Affiliated to Zhengzhou University from June 2017 to June 2020 were selected as the research object. Patients whose dental arch was mainly narrowed to the back were included in the spiral group, and the spiral arch expander was used for occlusion cushion type movable appliance, narrowing the front and back of the dental arch, and the patients with more obvious anterior narrowing were included in the fan-shaped group, using the fan-shaped expander occlusal cushion type movable appliance, each group with 31 cases, all underwent arch expansion treatment. The anterior tooth coverage, molar relationship, mandibular length, maximum transverse diameter of pear-shaped foramen, maximum transverse diameter of the lower part of the nasal cavity in the first premolar area, and upper alveolar seat angle (SNA), mandibular sagittal angle (SNB), difference between SNA angle and SNB angle (ANB), nasal airway volume, pharyngeal airway volume and minimum cross-sectional area of pharyngeal airway between the two groups were compared before and after treatment.Results:The anterior tooth coverage in the sector group after treatment was significantly lower than that before treatment and after treatment in the control group(n P<0.05); the molar relationship between the two groups after treatment was significantly lower than that before treatment, and the mandibular long group was more significant higher than that before treatment (n P<0.05). After treatment, the largest transverse diameter of the piriform foramen, the largest transverse diameter of the lower part of the nasal cavity in the first premolar area, and the largest transverse diameter of the lower part of the nasal cavity in the first molar area were significantly higher in the two groups after treatment (n P<0.05). The SNA and ANB in the two groups after treatment were significantly higher than those before treatment, and SNB was lower than that before treatment(n P<0.05). The nasal airway volume, pharyngeal airway volume, and oropharyngeal airway minimum cross-sectional area of the two groups after treatment were significantly higher than those before treatment(n P<0.05).n Conclusions:Occlusal splint appliance with spiral and fan-shaped arch expander in children with complete cleft lip and palate can expand the arch and improve the effect of orthodontic treatment.
