目的:分析血清同型半胱氨酸(Hcy)与胱抑素C(CysC)对早期2型糖尿病肾病的临床诊断价值。方法:选择2013年5月至2016年5月我院收治的早期糖尿病肾病患者60例为A组,选取同期我院收治的单纯糖尿病患者60例为B组,另选我院同期健康体检者60例为C组,检测三组的血清Hcy、CysC、血尿素氮(BUN)、血肌酐(Scr)、尿蛋白排泄率(UAER)。结果:与C组比较,A组和B组的BUN、Scr、UAER均升高(P<0.05),A组的UAER高于B组(P<0.05),但A组的BUN、Scr与B组比较,组间差异无统计学意义(P 0.05)。A组的病程明显高于B组(P<0.05)。与C组比较,A组Hcy与CysC水平升高(P<0.05),B组的Hcy水平升高(P<0.05),CysC水平升高,但差异无统计学意义(P 0.05);与B组比较,A组Hcy与CysC水平升高(P<0.05)。A组的Hcy、CysC、Hcy+CysC阳性率均明显高于B组和C组(P<0.05),B组Hcy、CysC、Hcy+CysC阳性率均明显高于C组(P<0.05);A组和B组的Hcy+CysC联合检测的阳性率高于Hcy、CysC的单独检测阳性率(P<0.05)。Hcy与病程、BUN、Scr、UAER均呈正相关,相关系数r=0.650、0.488、0.596、0.761,P<0.05,CysC与病程、BUN、Scr、UAER均呈正相关,相关系数r=0.681、0.502、0.601、0.825,P<0.05。结论:血清Hcy与CysC可及时、准确的反应2型糖尿病患者的肾损伤情况,从而有利于早期2型糖尿病肾病的及时检出。
Objective: To analyze the clinical value of serum homocysteine (Hcy) and cystatin C (CysC) in the diagnosis of early type 2 diabetic nephropathy. Methods: From May 2013 to May 2016, 60 patients with early diabetic nephropathy admitted to our hospital were selected as Group A. Sixty patients with simple diabetes mellitus admitted to our hospital from January 2013 to May 2016 were selected as group B, and another 60 In group C, serum Hcy, CysC, BUN, Scr and UAER were measured in three groups. Results: Compared with group C, the levels of BUN, Scr and UAER in group A and group B were significantly increased (P <0.05), while those in group A were higher than those in group B (P <0.05) There was no significant difference between groups (P> 0.05). The course of disease in group A was significantly higher than that in group B (P <0.05). The levels of Hcy and CysC in group A were significantly higher than those in group C (P <0.05), while the levels of Hcy and CysC in group B were significantly increased (P <0.05) Group A, Hcy and CysC levels increased (P <0.05). The positive rates of Hcy, CysC and Hcy + CysC in group A were significantly higher than those in group B and C (P <0.05). The positive rates of Hcy, CysC and Hcy + CysC in group B were significantly higher than those in group C (P <0.05). The positive rates of Hcy + CysC in group A and group B were higher than those of Hcy and CysC alone (P <0.05). There was a positive correlation between Hcy and duration of disease, BUN, Scr, UAER, correlation coefficient r = 0.650,0.488,0.596,0.761, P <0.05. CysC was positively correlated with duration, BUN, Scr and UAER, correlation coefficient was 0.681,0.502, 0.601, 0.825, P <0.05. Conclusion: Serum Hcy and CysC can timely and accurately reflect the renal injury in type 2 diabetic patients, which is beneficial to the early detection of type 2 diabetic nephropathy.
