目的探讨超声光散射成像系统(DOT)对各种病理类型乳腺病变的诊断价值。方法对我院行DOT检查的乳腺病变共329个进行回顾性研究,以病理诊断为金标准分为乳腺增生病变,良性肿瘤,炎性病变和恶性肿瘤,用非参数U检验对比各病理类型乳腺病变的SDI值。结果对照病理结果,329个病变中各种类型增生病变151个(45.90%),炎性病变16个(4.86%),各种类型良性肿瘤性病灶67个(20.36%),恶性病灶95个(28.88%)。各病理组间SDI值相比,炎性病变和恶性肿瘤之间差异无统计学意义(P=0.824),其余组间差异有统计学意义(P<0.05)。按增生和非增生病变分类根据ROC曲线获得SDI的最佳诊断阈值为105,灵敏度82%、特异度73%、准确度80.2%。结论 SDI值在各病理组间分布有一定特征,对区分增生性病变和非增生性病变有一定价值。
Objective To investigate the diagnostic value of ultrasonic light scattering imaging system (DOT) in various pathological types of breast lesions. Methods A total of 329 breast lesions underwent DOT in our hospital were retrospectively studied. The pathological diagnosis was divided into mammary gland hyperplasia lesions, benign tumors, inflammatory lesions and malignant tumors. Non-parametric U test was used to compare the pathological types of breast The SDI value of the lesion. Results According to the pathological results, 151 (45.90%) were all kinds of hyperplastic lesions in 329 lesions, 16 (4.86%) were inflammatory lesions, 67 (20.36%) were all kinds of benign neoplastic lesions and 95 were malignant lesions 28.88%). There was no significant difference between SDI values of pathological groups and inflammatory lesions and malignant tumors (P = 0.824). The difference between the other groups was statistically significant (P <0.05). According to the classification of hyperplastic lesions and non-proliferative lesions, the best diagnostic threshold of SDI obtained by ROC curve is 105 with sensitivity of 82%, specificity of 73% and accuracy of 80.2%. Conclusion The distribution of SDI in different pathological groups has certain characteristics, and has certain value in differentiating proliferative and non-proliferative diseases.
